Ar piekļuvi saistītas komplikācijas hemodialīzes pacientiem
2018
Ievads: ar asinsvadu pieeju saistītas komplikācijas tiek bieži novērotas hemodialīzes (HD) pacientiem, kuriem tās palielina komorbiditātes un nāves riskus. Mērķis: Izpētīt saslimstības un ar piekļuvi saistīto komplikāciju veidus Latvijas HD pacientiem, kā arī iespējamos riska faktorus un to saistību ar komplikācijām, lai veicinātu to novēršanu. Materiāli un metodes: Šis ir retrospektīvs aprakstošs pētījums, kura pamatā ir Paula Stradiņa Klīniskās Universitātes slimnīcas (PSKUS) HD nodaļas pacientu datu apkopojums. Pētījumā tika iekļauti visi hroniskie HD pacienti, kuri laika posmā no 2018. g. 26.februāra līdz 26. martam saņēma regulāru hronisku HD. Šo pacientu dialīzes piekļuves komplikācij…
Perioperative and periprocedural airway management and respiratory safety for the obese patient: 2016 SIAARTI Consensus.
2016
Proper management of obese patients requires a team vision and appropriate behaviors by all health care providers in hospital. Specialist competencies are fundamental, as are specific clinical pathways and good clinical practices designed to deal with patients whose Body Mass Index (BMI) is ≥30 kg/m2. Standards of care for bariatric and non-bariatric surgery and for the critical care management of this population exist but are not well defined nor clearly followed in every hospital. Thus every anesthesiologist is likely to deal with this challenging population. Obesity is a multisystem, chronic, proinflammatory disorder. Unfortunately many countries are facing a marked increase in the obese…
Familial hypercholesterolæmia in children and adolescents: Gaining decades of life by optimizing detection and treatment
2015
Contains fulltext : 155263.pdf (Publisher’s version ) (Open Access) Familial hypercholesterolaemia (FH) is a common genetic cause of premature coronary heart disease (CHD). Globally, one baby is born with FH every minute. If diagnosed and treated early in childhood, individuals with FH can have normal life expectancy. This consensus paper aims to improve awareness of the need for early detection and management of FH children. Familial hypercholesterolaemia is diagnosed either on phenotypic criteria, i.e. an elevated low-density lipoprotein cholesterol (LDL-C) level plus a family history of elevated LDL-C, premature coronary artery disease and/or genetic diagnosis, or positive genetic testin…
256 Functional analysis of elective nephron-sparing surgery versus radical nephrectomy for renal tumors larger than 4 cm
2012
Objective To preserve renal function, nephron sparing surgery (NSS) for renal tumors should be performed. Little is known about perioperative morbidity and long-term functional outcome of patients after elective NSS compared with radical nephrectomy (RN) in renal tumors >4 cm. Materials and Methods Eight-hundred twenty-nine patients were treated with either RN (n = 641) or NSS (n = 188) for renal tumors >4 cm. After pairing the cohort for age, grading, TNM, size, gender, and preoperative renal function and excluding patients with imperative indication and metastases, 247 patients remained for functional analysis. Serum creatinine (SCr) values were used to estimate glomerular filtration rate…
Large retroperitoneal abscess extended to the inferior right limb secondary to a perforated ileal Crohn’s disease: the importance of the multidiscipl…
2016
The typical complications of Crohn's disease concerns small and large bowel. The full thickness inflammation of the intestinal wall develops in strictures, fistulas and abdominal abscesses. Nowadays the most accepted therapeutic for intra-abdominal abscess option is antibiotic therapy and, in case of need, percutaneous drainage of the abscess. If the abscess passes through the pelvic foramen the abscess can involve the inferior limbs. We report a case a perforation of terminal ileum in Crohn's disease complicated by a large abscess of the right iliac fossa reaching the spaces between the anterior lateral muscles of the right thigh as far as the anterior lateral pre-tibial region. We discuss…
MDCT Enteroclysis and Enterography
2015
Since the first studies suggesting its possible role in the management of Crohn’s disease patients, multidetector computed tomography has usually been considered a useful diagnostic technique in Crohn’s disease for the detection of extraenteric complications, as intra-abdominal abscesses, the study of strictures, prestenotic dilatations, fistulas, and postsurgical complications. Moreover, it is a widely available technique and is considered the gold standard in emergency.
Relative incidence and mortality of congenital heart defects diagnosed by angiohemodynamic methods: a 17-year study.
1992
Over a 17-year period (January 1971 to January 1988), 2322 children, aged 0-14 years, were diagnosed as having congenital heart disease (CHD) by cardiac catheterization and angiography. Excluding those with highly complex or undiagnosed defects, there were 2156 children with CHD, 72.4% of whom were treated surgically, with a total surgical mortality rate of 24.1%. After a mean follow-up of 9 years the overall mortality of the cohort was 29.9%, 29.1% occurring in the first month of life, 39.6% between 1 month and 1 year, and 31.2% between 1 and 14 years. The incidence, mortality, and age at death of each cardiac defect are presented and compared with the results of other studies. The overall…
Medico-legal implications of sleep apnoea syndrome: driving license regulations in Europe.
2008
Background: Sleep apnoea syndrome (SAS), one of the main medical causes of excessive daytime sleepiness, has been shown to be a risk factor for traffic accidents. Treating SAS results in a normalized rate of traffic accidents. As part of the COST Action B-26, we looked at driving license regulations, and especially at its medical aspects in the European region. Methods: We obtained data from Transport Authorities in 25 countries (Austria, AT; Belgium, BE; Czech Republic, CZ; Denmark, DK; Estonia, EE; Finland, FI; France, FR; Germany, DE; Greece, GR; Hungary, HU; Ireland, IE; Italy, IT; Lithuania, LT; Luxembourg, LU; Malta, MT; Netherlands, NL; Norway, EC; Poland, PL; Portugal, PT; Slovakia,…
