Cusp overlap technique for transcatheter self-expanding aortic valve implantation

2021

Transcatheter aortic valve implantation (TAVI) may cause atrioventricular conduction disturbances and, consequently, the necessity for permanent pacemaker implantation (PPI). This is because the His bundle surfaces at the basal part of the membranous septum (in the neighbourhood of the non-coronary cusp) and implant depth greater than the length of the membranous septum may interfere with the conduction system [1]. To mitigate this risk, the valve should be implanted at a high position to minimize the contact of the valve frame with the base of membranous septum. During TAVI, the valve is usually positioned in a coplanar fluoroscopic projection with 3 cusps view, but for a self-expanding va…

Geoarchaeological study of the Phoenician cemetery of Tyre-Al Bass (Lebanon) and geomorphological evolution of a tombolo

2008

The geoarchaeological record of the Phoenician necropolis of Al Bass (Lebanon) provides information concerning the geomorphological evolution of a late Holocene tombolo. Physical and chemical analysis of sediments indicates that the cemetery (9th century B.C.) was located near a littoral lagoon, between the dunes of a cuspate spit pointing toward the island of Tyre. From the sea apex of this spit, the moles mentioned in historical chronicles were constructed. Once mainland and island were connected, at the northern coast (where the port of Sidon was located), a sediment trap was formed, which quickly filled with silt. Afterwards, an extensive field of sand dunes buried all the archaeologica…

Intraoperative Evaluation of the Reconstruction of the Atrioventricular Valves by Means of Transesophageal Echocardiography

1989

In 44 operations, the mitral valve was reconstructed in 28 patients, the tricuspid valve in 12, and both atrioventricular (AV) valves in four. The results of reconstruction were tested first in the open arrested heart by injection of a crystalloid solution into the left or right ventricle, either through the reconstructed valve or via one of the great arteries. Upon termination of cardiopulmonary bypass the valve function was examined in the beating heart by transesophageal contrast echocardiography (TEE). For purposes of visualization, 0.5–1.0 cc of agitated Gelifundol was injected into the ventricle. In 31 patients, open testing and echocardiographic visualization showed identical, good o…

Computed tomography imaging needs for novel transcatheter tricuspid valve repair and replacement therapies

2020

Abstract Transcatheter tricuspid valve therapies are an emerging field in structural heart interventions due to the rising number of patients with severe tricuspid regurgitation and the high risk for surgical treatment. Computed tomography (CT) allows exact measurements of the annular plane, evaluation of adjacent structures, assessment of the access route, and can also be used to identify optimal fluoroscopic projection planes to enhance periprocedural imaging. This review provides an overview of current transcatheter tricuspid valve repair and replacement therapies and to what extent CT can support these interventions.

Tricuspid valve repair with the Cardioband system: two-year outcomes of the multicentre, prospective TRI-REPAIR study

2021

AIMS: Tricuspid regurgitation (TR) is associated with high morbidity and mortality rates with limited treatment options. We report one- and two-year outcomes of the Cardioband tricuspid valve reconstruction system in the treatment of ≥moderate functional TR in the TRI-REPAIR study. METHODS AND RESULTS: Thirty patients were enrolled in this single-arm, multicentre, prospective study. Patients were evaluated as having ≥moderate, symptomatic functional TR and deemed inoperable due to unacceptable surgical risk. Clinical, functional, and echocardiographic data were prospectively collected up to two years (mean duration 604±227 days). At baseline, 83% were in NYHA Class III-IV, and the mean LVEF…

Regional variation of wall shear stress in ascending thoracic aortic aneurysms.

2014

The development of an ascending thoracic aortic aneurysm is likely caused by excessive hemodynamic loads exerted on the aneurysmal wall. Computational fluid-dynamic analyses were performed on patient-specific ascending thoracic aortic aneurysms obtained from patients with either bicuspid aortic valve or tricuspid aortic valve to evaluate hemodynamic and wall shear parameters, imparting aneurysm enlargement. Results showed an accelerated flow along the outer aortic wall with helical flow in the aneurysm center for bicuspid aortic valve ascending thoracic aortic aneurysms. In a different way, tricuspid aortic valve ascending thoracic aortic aneurysms exhibited normal systolic flow without su…

Cone-beam computed tomography analysis of root and canal morphology of mandibular premolars in a Spanish population

2014

Purpose: This study aimed to investigate the clinical anatomy of lower premolar roots in a Spanish population by using cone-beam computed tomography (CBCT), correlating findings with patient gender and tooth type. Materials and Methods: Using 70 CBCT images, we evaluated 126 healthy, untreated, well-developed lower premolars. The number and morphology of roots and root canals, and the foramina number were assessed. Results for gender and tooth type were compared using the chi-squared and ANOVA tests. Results: The average length of teeth and roots was significantly higher in men (p=0.00). All 126 premolars had a single root. One canal was found in 83.3% of the premolars, with no gender or to…

Effect of saliva contamination on bracket failure with a self-etching primer: A prospective controlled clinical trial

2010

Introduction The aim of this study was to evaluate in vivo the effect of saliva contamination at different stages of the bonding procedure on the bond failure rate and the adhesive remaining on teeth after debonding brackets bonded with a hydrophilic self-etching primer (Transbond Plus self-etching primer [TSEP], 3M Unitek, Monrovia, Calif). Methods This was a prospective controlled clinical trial. The sample consisted of 46 patients with similar treatment plans and mechanotherapies. Stainless steel brackets (n = 531) were bonded with TSEP. The patients were divided into 2 groups: contamination with saliva before TSEP application and contamination with saliva after TSEP application. In both…
