Discover cervical disc arthroplasty versus anterior cervical discectomy and fusion in symptomatic cervical disc diseases: A meta-analysis.

2016

OBJECTIVE:Symptomatic cervical disc disease (SCDD) is a common degenerative disease, and Discover artificial cervical disc, a new-generation nonconstrained artificial disk, has been developed and performed gradually to treat it. We performed this meta-analysis to compare the efficacy and safety between Discover cervical disc arthroplasty (DCDA) and anterior cervical discectomy and fusion (ACDF) for SCDD. METHODS:An exhaustive literature search of PubMed, EMBASE, and the Cochrane Library was conducted to identify randomized controlled trials that compared DCDA with ACDF for patients suffering SCDD. A random-effect model was used. Results were reported as standardized mean difference or risk …

Early Onset of Guillain–Barré Syndrome Following Lumbar Disc Herniation Surgery: An Unexpected Clinical Evolution

2021

Letter: Guillain–Barré syndrome (GBS), is a neurologic complication rarely reported following a spinal surgery procedure.1, 2, 3, 4, 5, 6 GBS is a potentially fatal, immune-mediated disease of the peripheral nerves and nerve roots that is usually triggered by infections. It is the most common cause of acute flaccid paralysis, with an annual global incidence of approximately 1–2 per 100,000 person-years.7 Although the clinical presentation of the disease is heterogeneous, patients typically present with weakness and sensory signs in the legs that progress to the arms and cranial muscles. Disease progression can be rapid in approximately 20% of patients with respiratory failure requiring mech…

Computed Tomography Findings Associated with Clinical Outcome After Dynamic Posterior Stabilization of the Lumbar Spine.

2016

Objective To evaluate whether preoperative multirow detector computed tomography (MDCT) findings were associated with clinical outcome 24 months after dynamic stabilization for painful degenerative lumbar spine disease. Methods Preoperative MDCT examinations of 63 patients (66 ± 11.7 years; 60% women) treated with a dynamic screw rod system for painful degenerative segmental instability with/without spinal stenosis were assessed for quantitative and qualitative parameters defining degenerative changes of the thoracolumbar spine, including grades of disc herniation, degenerative spondylolisthesis, vertebral body sclerosis, cross-sectional area of the spinal canal at disc level, intervertebra…

ADDISC lumbar disc prosthesis: Analytical and FEA testing of novel implants

2023

The intact intervertebral disc is a six-freedom degree elastic deformation structure with shock absorption. 'Ball-and-socket' TDR do not reproduce these properties inducing zygapophyseal joint overload. Elastomeric TDRs reproduce better normal disc kinematics, but repeated core deformation causes its degeneration. We aimed to create a new TDR (ADDISC) reproducing healthy disc features. We designed TDR, analyzed (Finite Element Analysis), and measured every 500,000 cycles for 10 million cycles of the flexion-extension, lateral bending, and axial rotation cyclic compression bench-testing. In the inlay case, we weighted it and measured its deformation. ADDISC has two semi-spherical articular s…

Biomechanical Study of the Osteoporotic Spine Fracture: Optical Approach

2021

Background and objectives: Osteoporotic spine fractures represent a significant factor for decreasing quality of life in the elderly female population. Understanding the mechanisms involved in producing these fractures can improve their prevention and treatment. This study presents a biomechanical method to produce a vertebral fracture, conducted on a human spine segment, observing the displacements and strains in the intervertebral disc, endplate, and vertebral body. Materials and Methods: We performed two tests, one corresponding to an extension loading, and the second to an axial loading. Results: The maximum displacement in the target vertebral body presented higher values in the case o…

Pro-inflammatory, Pleiotropic, and Anti-inflammatory TNF-α, IL-6, and IL-10 in Experimental Porcine Intervertebral Disk Degeneration

2009

The aim of this study was to check the balance between tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), and interleukin-10 (IL-10) in well-developed end-stage disk disease in the disk itself as well as in paradiskal spine. In 6 domestic pigs the cranial bony end plate of the L4 vertebra was perforated to the nucleus pulposus. At 3 months the degenerated experimental and contiguous control disks, together with the adjoining bony and cartilaginous vertebral end plates, bone marrow, and spinal ligaments, were excised and used for immunohistochemical analysis. In general, there were more TNF-α and in particular IL-10 positive cells in the degenerated disks than in the control disks, where…

Reoperations After First Lumbar Disk Herniation Surgery with or without Implantation of Mechanical Annular Closure Device

2019

Lumbar diskectomy is a widely performed surgical procedure in patients affected by lumbar disk herniation (LDH). Such a surgical procedure is considered to be safe and able to provide a consistent symptomatic relief in most patients. However, perioperative complications have been reported, ranging from 13% to 15%, and pose a considerable financial burden among patients, hospitals, and national health care agencies. The most common perioperative complication following lumbar diskectomy for disk herniation is the recurrence. Although the recurrence rate of LDH has been reported to be 5%−15%, its exact incidence may be underestimated. Recurrent lumbar disk herniation (rLDH) is defined as disk …

Financial Aspects of Cervical Disc Arthroplasty: A Narrative Review of Recent Literature

2020

Recently, there has been significant interest in understanding the cost-effectiveness of treatments in spine surgery as health care systems in the United States move toward value-based care and alternative payment models. Previous studies have shown comparable outcomes of cervical disc arthroplasty (CDA) and anterior cervical discectomy fusion; however, there is a lack of consensus on the cost-effectiveness of CDA to support full adoption. Evidence of the limitations of these cost-analysis studies also exists in the literature, including industry funding, potential selection bias, and varying methods of calculating value. The goal of this narrative review is to provide an overview of the co…

Mid- to Long-Term Outcomes of Cervical Disc Arthroplasty versus Anterior Cervical Discectomy and Fusion for Treatment of Symptomatic Cervical Disc Di…

2015

Purpose This study aimed to investigate the mid- to long-term outcomes of cervical disc arthroplasty (CDA) versus anterior cervical discectomy and fusion (ACDF) for the treatment of 1-level or 2-level symptomatic cervical disc disease. Methods Medline, Embase, and the Cochrane Central Register of Controlled Trials databases were searched to identify relevant randomized controlled trials that reported mid- to long-term outcomes (at least 48 months) of CDA versus ACDF. All data were analyzed by Review Manager 5.3 software. The relative risk (RR) and 95% confidence intervals (CIs) were calculated for dichotomous variables. The weighted mean difference (WMD) and 95%CIs were calculated for conti…
