Die fiberoptische Intubation neurochirurgischer Patienten*

1993

Patients with cervical spine injury presenting with respiratory distress require airway management that does not compromise integrity of the atlanto-occipital joint. Endotracheal intubation by means of direct laryngoscopy is not suitable. The method of choice is nasotracheal intubation of the awake patient, using a flexible fibre bronchoscope. If anatomy or surgical access render the nasal approach impossible, fibre optic intubation can be performed orotracheally, utilising specific technical aids. Flexible fibrescopes are available in different sizes (length and diameter): selection is base on the patient's anatomical requirements. Aids to orotracheal intubation are constructed with a bore…

On the Role of Inequalities in Legal Systems: A Tocquevilian View

2008

The present paper proposes to interpret the differences in legal systems between common-law and civil-law nations as arising from the importance given to adjudication in comparison with statute laws. It focuses on the relative costs of legal change by adjudication (case law development) when compared with legislation (statutory law development). The main argument is that the public concern with equality is a major determinant of the relative cost of adjudication in a legal system. We develop a model of the legal process that illustrates Tocqueville's fundamental intuition with regard to the uniformity of legal rules, and as a consequences, the relative importance of adjudication and legisla…

What Can Plans Do for Legal Theory?

2012

In his book, Legality (2011), Scott Shapiro puts forward what he claims to be "a new, and hopefully better" (better, namely, than the ones given so far) answer to "the overarching question of ‘What is law?’ - The central claim of this new account - the "Planning Thesis" - is that "legal activity is a form of social planning" -. "Legal institutions plan for the communities over which they claim authority, both by telling members what they may or may not do, and by identifying those who are entitled to affect what others may or may not do. Following this claim, legal rules are themselves generalized plans, or planlike norms, issued by those who are authorized to plan for others. And adjudicat…

Bene : Adverb or noun?

2013

International audience; When Italian bene ‘good / well’ occurs with fare ‘do / make’, several constructs with remarkably different argument frames are involved. This paper deals with three of them: (a) Il latte fa bene ai bambini ‘Milk is good for children’; (b) Fa bene il suo lavoro ‘She does her job well’, and (c) Faresti bene a non dire niente ‘You would do well to say nothing about it’. We discuss dictionary discrepancies concerning the lexical category of 'bene' in (a), which we take to be a noun predicate, and draw a distinction between the adverbial uses in (b) and (c).

2-Organoselenomethyl-1H-benzimidazole Complexes of Copper(II) and Copper(I)

2009

Structurally characterized 2-methylselenomethyl-1H-benzimidazole (msbi) is shown to form complexes in 2:1 ratio with copper(II). Whereas the central cation [Cu(msbi)2Cl]+ = 1+ in [Cu(msbi)2Cl]Cl·2MeOH crystallizes in an approximately mixed square-pyramidal/trigonal-bipyramidal structure (τ = 0.52) with the second and third row atoms chlorine and selenium in the equatorial plane, the related [Cu(msbi)2(MeOH)](BF4)2 = [2](BF4)2 exhibits a closer to square-pyramidal arrangement with τ = 0.36 for that complex dication 22+. In both cases, the Cu–N bonds are short at about 1.94 A, whereas the Cu–Se distances vary between 2.62 A for 1+ and ca. 2.54 A for 22+. Slight differences between the Jahn–Te…

Adverse drug reactions to antiretroviral medication

2009

Antiretroviral therapy has greatly improved prognosis of HIV infection, with a dramatic reduction of morbidity and mortality worldwide. Nevertheless, the condition is still a common cause of death in many underdeveloped countries, where effective treatment is not always unavailable. More than 20 drugs active against HIV are commercially available, which belong to one of four groups: nucleoside reverse transcriptase inhibitors, non-nucleoside reverse transcriptase inhibitors, protease inhibitors, and fusion/entry inhibitors. In the near future new drugs are expected, including those of a novel group, the integrase inhibitors. To avoid viral resistance, combinations of the drugs must always b…

Hepatocellular carcinoma recurrence in patients with curative resection or ablation: impact of HCV eradication does not depend on the use of interfer…

2016

none 48 no Background: In HCV-infected cirrhotic patients with successfully treated early hepatocellular carcinoma (HCC), the time to HCC recurrence and the effects of sustained viral eradication (SVR) by interferon (IFN)-based or IFN-free regimens on HCC recurrence remain unclear. Aim: To perform an indirect comparison of time to recurrence (TTR) in patients with successfully treated early HCC and active HCV infection with those of patients with SVR by IFN-based and by IFN-free regimens. Methods: We evaluated 443 patients with HCV-related cirrhosis and Barcelona Clinic Liver Cancer Stage A/0 HCC who had a complete radiological response after curative resection or ablation. Active HCV infec…

Prospective evaluation of hepatic steatosis in HIV-infected patients with or without hepatitis C virus co-infection

2012

Background: Limited data are available on hepatic steatosis (HS) in HIV patients who are not infected with hepatitis C virus (HCV). The aims of this study were to assess the prevalence of HS and its risk factors in HIV patients with and without HCV infection, and to evaluate whether HS correlates with advanced liver fibrosis and/or cardiovascular disease risk. Methods: Fifty-seven HIV mono-infected and 61 HIV/HCV co-infected patients were enrolled consecutively. All patients underwent liver ultrasound and transient elastography. The main parameters of liver function, HIV and HCV viral loads, CD4+ cell counts, and data on highly active antiretroviral therapy (HAART) were recorded. Cardiovasc…
