Spinal cord monitoring during intraspinal extramedullary tumor operations (Peroneal nerve evoked responses)

Longterm scalp recording of early SEP components triggered by peroneal or tibial nerve stimulation detects functional disturbances of spinal cord transmission due to mechanical trauma. We confirm previous observations that preoperative SEP patterns reflect neurological deficits and clearly show functional disturbances even on the side where they are not manifest. Peroneal nerve SEP have a well-known P40-peak corresponding to activities of neurons at the postcentral cortical layers. The P40-peak was identified in only 55% of our recordings. Age- and gender-matched reference values are essential for the clinical use of quantitative sensory testing (QST). To extend the standard test sites for QST-according to the German Research Network on Neuropathic Pain-to the trunk, we collected QST profiles on the back in 162 healthy subjects. Sensory profiles for standard test sites were within normal interlaboratory differences. QST revealed lower sensitivity on the upper back than the hand, and higher sensitivity on the lower back than the foot, but no systematic differences between these trunk sites. Clinical evaluation criteria for the assessment of impaired pain sensitivity by thulium-laser evoked potentials

Abstract Objectives : Cortical potentials evoked by carbon dioxide laser pulses have been applied in clinical practice to study nociceptive pathways for several years. In this study, we evaluate the properties of an infrared laser (thulium-YAG) with a penetration depth in the skin that matches the intracutaneous depth of nociceptors. Methods : Temperature measurements and modelling showed that the thulium laser generates painful intracutaneous temperatures with less surface heating than the carbon dioxide laser and with no side effects (up to 600 mJ pulse energy). Pancreatic encephalopathy: a 7-year follow-up case report and review of the literature

Pancreatic encephalopathy is a rare complication of acute pancreatitis. Clinical features include focal neurological signs and acute onset of dementia. This picture can fluctuate over time: cyclic progression with remission and relapses has been described. We present the case of a 43-year-old man who, after an acute episode of pancreatitis, experienced five relapses, with alternating focal signs. The patient has improved, but cognitive impairment persists after a 7-year follow-up.

The pterygoid reflex in man and its clinical application

A technique for eliciting and recording the stretch reflex (R) of the medial pterygoid muscle (Pter) is described. The latency was 6.9 ± 0.43 ms in 23 healthy volunteers (mean age 23.7 years) showing a side-to-side difference of 0.29 ± 0.21 ms. The PterR latencies were little shorter and side-to-side difference little greater than of the masseter reflex. Observations in 5 selected patients with small brainstem lesions suggest that the neurons of the PterR afferents form a cluster within the caudal portion of the trigeminal mesencephalic nucleus. The nonlinear relation between stimulus intensity and response amplitude of the electroretinogram (ERG) scotopic b wave can be described by a curve based on the Naka-Rushton (NR) equation. Epileptic seizures and cerebrovascular disease

- A series of 88 patients with completed stroke was selected in which heralding, early and late seizures were distinguished according to their onset. Relationships between CT scan and clinical EEG data are discussed with particular emphasis on possible mechanisms of seizures. Thus, small emboli or haemodynamic factors are stressed in the cases of heralding seizures, metabolic disturbances with cytotoxic effects in early seizures and chronic epileptic focus in late seizures.
