Struktur- und Ergebnisqualit�t eines Rettungssystems

Ziel dieser Untersuchung war es zu klaren, ob die Struktur des Mainzer Rettungssystems eine flachendeckende und fristgerechte Versorgung von Notfallpatienten gewahrleistet. Daruber hinaus wurde der Einflus infrastruktureller Faktoren des Stadtgebietes sowie der Reanimation durch Ersthelfer auf Struktur- und Ergebnisqualitat untersucht. Das zu untersuchende System versorgt 190 000 Einwohner auf einer Flache von 80 km2 mit Hilfe von 4 Rettungs- (RTW) und 1 Notarztwagen (NAW), die in einem gestaffelten System mit den Moglichkeiten eines Fruhdefibrillationsprogramms fur Rettungsassistenten eingesetzt werden.

In der vorliegenden Arbeit werden Publikationen zur Effektivitat von primaren Entgiftungsmasnahmen kritisch analysiert. Die induzierte Emesis erbringt gegenuber der Therapie mit aktivierter Kohle keine Vorteile und scheint verzichtbar zu sein. Die Darmspulung konnte eine interessante Alternative darstellen, wenn deren Bedeutung noch evaluiert werden kann. Die vorliegenden Untersuchungen unterstreichen den hohen Stellenwert einer fruhzeitigen Kohletherapie (unter Berucksichtigung der Kontraindikationen). Perorale Intoxikationen mit Betablockern Eine klinisch-epidemiologische Studie und Stand der Therapie

Vergiftungen mit Betablockern sind intensivmedizinisch relevant und imponieren vorwiegend mit Bradykardie, Hypotonie und neurologischen Symptomen. Vorliegende Arbeit untersucht 244 Intoxikationen mit Betablockern aus den Jahren 1986 bis 1996. Die Auswertung wurde anhand des speziellen follow up-Systems der Beratungsstelle bei Vergiftungen der Universitatsklinik Mainz vorgenommen. Im Kindesalter ist bei akzidentell eingenommenen Mengen von 1–2 Einzeldosen (insg. 320 mg) eines Betablockers nur mit geringen oder keinen Symptomen zu rechnen und hinsichtlich primarer Giftentfernung (Erbrechen, Magenspulung) Zuruckhaltung zu empfehlen. La mise en œuvre de la sédation profonde et continue maintenue jusqu’au décès en réanimation peut s’avérer complexe car il faut tenir compte de la singularité de chaque patient et de son entourage. Cet aspect de la prise en charge devrait être évoqué autant que possible avec le patient et ses proches bien avant que la question de la fin de vie ne se pose. Formation des soignants, information des patients et des proches et adaptation de l’environnement en fin de vie représentent les pistes d’amélioration de la prise en charge de la fin de vie en réanimation.

Objective: The present study investigated the impact of the vascular access site for cardiac output (CO) measurement by thermodilution on survival and neurohistopathological injury in a rat model of cardiac arrest (CA) and cardiopulmonary resuscitation (CPR). Effect of active compression–decompression resuscitation (ACD-CPR) on survival: a combined analysis using individual patient data

Active compression decompression resuscitation (ACD-CPR) has been developed as an alternative to standard cardiopulmonary resuscitation (S-CPR). To determine the effect of ACD-CPR on survival and neurologic outcome in patients with out-of-hospital cardiac arrest, this combined analysis involved individual patient data from 2866 patients from seven separate randomized prospective prehospital studies who had received ACD-CPR or S-CPR after out-of-hospital cardiac arrest in seven international sites. Small volume resuscitation with tempol is detrimental during uncontrolled hemorrhagic shock in rats.

In a previous study, titration of a hypertonic saline (HTS) solution during severe uncontrolled hemorrhagic shock (UHS) failed to reduce mortality. In a separate study, a novel antioxidant, polynitroxylated albumin (PNA) plus tempol (4-hydroxy-2,2,6,6-tetramethylpiperidine-N-oxyl), infused during shock increased long-term survival. Titrated hypertonic/hyperoncotic solution for hypotensive fluid resuscitation during uncontrolled hemorrhagic shock in rats.

In volume- or pressure-controlled hemorrhagic shock (HS) a bolus intravenous infusion of hypertonic/hyperoncotic solution (HHS) proved beneficial compared to isotonic crystalloid solutions. During uncontrolled HS in animals, however, HHS by bolus increased blood pressure unpredictably, and increased blood loss and mortality.
