Use of anti-epidermal growth factor receptor (anti-EGFR) agents has yielded significant advances in the treatment of patients with metastatic colorectal cancer. In fact these drugs, which include the monoclonal antibodies cetuximab and panitumumab, can be delivered both as a single agent and in combination with chemotherapy, achieving better survival and quality of life and in some cases also resectability of metastases. However, these agents can result in the development of toxicities that are usually different from those observed with chemotherapy alone. The article summarizes the expert discussion and recommendations on the use of molecular markers and of biological targeted therapies in metastatic colorectal cancer (mCRC), as well as a proposed treatment decision strategy for mCRC treatment. The meeting was conducted during the 11th ESMO/World Gastrointestinal Cancer Congress (WGICC) in Barcelona in June 2009. The manuscript describes the outcome of an expert discussion leading to an expert recommendation. Introduction This randomized phase II study investigated pemetrexed in combination with the epidermal growth factor receptor (EGFR)-targeting monoclonal antibody matuzumab compared with pemetrexed alone as second-line therapy for patients with advanced non-small cell lung cancer. Methods Patients received pemetrexed 500 mg/m 2 every 3 weeks either alone ( n = 50) or in combination with matuzumab at either 800 mg weekly ( n = 51) or 1600 mg every 3 weeks ( n = 47). Individualized Tumor Therapy: Biomarkers and Possibilities for Targeted Therapy with Natural Products

Although many tumors respond to chemotherapy, not all patients benefit from anticancer drugs. Tumors often develop resistance to drugs and concentrations sufficient to eradicate the cancer cannot be used due to the severe side effects of chemotherapy. In the present chapter, we give an overview of research on biomarkers with prognostic and predictive value and summarize our own efforts in this context. With a battery of biomarkers and corresponding synthetic and natural targeted drugs, it is likely that custom-tailored combination treatments will soon become a reality and that each individual cancer patient will be treated based on his or her individual molecular tumor architecture.
