The effect of cement fixation on initial micromotion of the femoral component in condylar knee replacement

Four fresh, cadaveric specimens were fitted with total knee replacements and mounted on a knee joint loading and measuring rig. Femoral component micromotion in the sagittal plane was measured with a transducer during flexion and extension of the specimen. Various condylar designs were used without and with cement. Movement followed a characteristic curve pattern and was less with cement fixation particularly for bone of low trabecular strength as determined by bone sample tests for stiffness. Cement fixation appears to be superior to cementless fixation.

Systemic juvenile idiopathic arthritis (sJIA) is an autoinflammatory disease characterised by fever and arthritis. We describe the case of a 14-year-old girl hospitalised with fever associated with rash, myalgia, arthralgia and polyarticular involvement. Examinations revealed increased levels of C-reactive protein, erythrocyte sedimentation rate, ferritin, triglycerides, leukocytes, neutrophils, lactate dehydrogenase, fibrinogen, aspartate aminotransferase (AST), alanine aminotransferase (ALT) and γ-glutamyl transferase (GGT). We report seven patients with immune-mediated rippling muscle disease (iRMD) and AChR-antibody positive myasthenia gravis (MG) without germline caveolin-3 gene mutations. We describe the follow-up of two patients and the clinical features of five new patients (1 female, 4 male, aged 32 to 69 years). These presented with significant generalized, exercise-induced and electrically-silent muscle rippling with myalgia, combined with generalized MG. In two of the seven patients, MG appeared before iRMD. Mediastinal imaging excluded thymic alterations in all, although two had other coincident tumours. A novel myopathy characterized by hexagonally cross-linked tubular arrays has been reported in five patients. We studied the clinical and histopathological features of five additional unrelated patients with this myopathy. Patients experienced exercise intolerance with exercise-induced myalgia and weakness, without rhabdomyolysis. One patient additionally presented mild permanent pelvic girdle muscle weakness. Age at onset varied between 13 and 56 years. Medusa esiintyy antiikin mytologiassa käärmehiuksisena naishirviönä, gorgona, jonka kivettävää katsetta rohkeimmatkin miehet pelkäävät. Medusan myytissä esille tulevaa naiskuvaa on käsitelty sekä taiteentutkimuksessa että filosofiassa. Feministifilosofi ja -kirjailija Hélène Cixous (s. 1937) tekee myytistä ironisen tulkinnan, jossa Medusa nauraa häntä vastaan taistelevien miesten turhalle pelolle. Muutamia poikkeuksia lukuun ottamatta ironian ja feministisen filosofian välisiä yhteyksiä ei kuitenkaan ole selvitetty. Mitä Medusan myytti ja ironia tarjoavat feministiselle yhteiskuntakritiikille?
