Biophysics of venous return from the brain from the perspective of the pathophysiology of chronic cerebrospinal venous insufficiency

This article discusses the biophysical aspects of venous outflow from the brain in healthy individuals and in patients with chronic cerebrospinal venous insufficiency. Blood flows out of the brain differently, depending on body position. In the supine position it flows out mainly through internal jugular veins, while in the upright position it uses the vertebral veins. This phenomenon is probably not due to the active regulation of the flow but instead results from the collapse of jugular veins when the head is elevated. Muscarinic mobilization of choline in rat brain in vivo as shown by the cerebral arterio-venous difference of choline.

In anesthetized rats, the choline levels of cerebrospinal fluid and plasma obtained from blood collected from peripheral vessels (carotid artery, cardiac vessels) and from the transverse sinus were determined with a radioenzymatic assay. Cortical release of choline was studied using the "cup technique." The plasma choline level of the peripheral blood (11.5 mumol/L) was lower than that of the sinus blood. The resulting cerebral arterio-venous difference of choline was negative (3.2 mumol/L) and reflected the net release of choline from the whole brain. Rheological and metabolic leucocyte determinants in diabetes mellitus

In diabetics of type I and 2 we examined, in resting white blood cells (WBC), the filtration parameters (Initial Relative Flow Rate - lRFR, Clogging Rate - CR) employing the St. George Filtrometer, the polymorphonuclear cells (PMN) membrane fluidity, the PMN cytosolic Ca2+ content and the PMN membrane cholesterol/phospholipid ratio (C/PL). From the obtained data, it is evident that, while the lRFR of unfractionated WBC distinguishes normals from diabetics of type 1 and 2, the fIltration parameters of the PMN and mononuclear cells (MN) do not show any significant difference. Granulocyte colony-stimulating factor (G-CSF) induces a transient mobilization of hematopoietic progenitor cells from bone marrow to peripheral blood. Our aim was to evaluate safety of repeated courses of G-CSF in patients with amyotrophic lateral sclerosis (ALS), assessing disease progression and changes in chemokine and cytokine levels in serum and cerebrospinal fluid (CSF). Twenty-four ALS patients entered an open-label, multicenter trial in which four courses of G-CSF and mannitol were administered at 3-month intervals. Summary Hydroxyethyl starch (HES) is widely used for fluid management in broad populations of patients, particularly in the surgery and intensive care settings. Pruritus, often manifested as pruritic crises, is increasingly being recognized as a common major adverse effect of HES administration.
