Endocrine tumours: epidemiology of malignant digestive neuroendocrine tumours.

Little is known about patients with malignant digestive neuroendocrine tumours (MD-NETs). Although their incidence is increasing, MD-NETs remain a rare cancer, representing 1% of digestive cancers. Most MD-NETs are well-differentiated. MD-NET poorly differentiated carcinomas account for 20% of cases on average. Anatomical localisation of MD-NETs varied according to geographic region. Stage at diagnosis and prognosis for patients with MD-NETs in the general population are considerably worse than often reported from small hospital case series. Mutations in Patched (PTCH) have been associated with tumors characteristic both for children [medulloblastoma (MB) and rhabdomyosarcoma (RMS)] and for elderly [basal cell carcinoma (BCC)]. The determinants of the variability in tumor onset and histology are unknown. We investigated the effects of the time-point and dosage of Ptch inactivation on tumor spectrum using conditional Ptch-knockout mice. Under certain conditions, the fungal flora may overgrow, resulting in lesions of the digestive mucosa which, rarely, can have a local diffusion and/or spread to the lympho-hematogenous system. Endoscopic surveillance in ulcerative colitis: smart biopsies do it better.

Patients with ulcerative colitis (UC) have a significantly higher risk for the development of colitisassociated colorectal cancer. Endoscopic surveillance is recommended. The growth pattern of dysplastic tissue is often multifocal and flat. Thus, significant lesions might be overlooked. Multiple random biopsies are recommended, but cannot eliminate the possibility of missed cancers with severe consequences for the patient. Chromoendoscopy with methylene blue or indigo carmine can unmask flat lesions and may solve this problem. We describe a further, lethal case of protracted diarrhoea in a child from a family in whom 2 previous siblings died of protracted diarrhoea at 2 and 11 months. The patient, a boy, was born at term weighing 2.9 kg and was admitted at 6d with profuse watery diarrhoea, severe dehydration and metabolic acidosis.
