It is well established that cancer arises in chronically inflamed tissue, and this is particularly notable in the gastrointestinal tract. Classic examples include Helicobacter pylori-associated gastric cancer, hepatocellular carcinoma, and inflammatory bowel disease-associated colorectal cancer. Growing evidence suggests that these associations might be not casual findings. Focusing on individual cytokines has generated evidence that anti-inflammatory cytokine interleukin (IL)-10 and transforming growth factor-beta1 (TGF-beta1) may have a complex role in gastrointestinal carcinogenesis. Abstract Histamine has an important role in the process of the gastric mucosa inflammation acting via histamine receptor H2 (encoded by the gene HRH2). Single nucleotide polymorphism of the enhancer element of HRH2 gene promoter rs2067474 (1018G>A)may be associated with changes of expression of the receptor. We attempted to clarify the association of this polymorphism with gastric cancer and/or atrophic gastritis in the Latvian (Caucasian) population. Is Helicobacter pylori status relevant in the management of GORD?

Summary There is growing interest in the relationship between H. pylori infection and gastro-oesophageal reflux disease (GORD). However, this relationship is complex, as yet not fully elucidated, and probably based on a multiplicity of factors. The prevalence of H. pylori infection in patients with GORD is similar, more often lower than in matched controls. There is a negative correlation between H. pylori infection and the severity of GORD. There are many hypothetical mechanisms by which H. pylori infection may protect from the development of GORD. Advantage of eradication therapy for Helicobacter pylori before kidney transplantation in uremic patients.

ABSTRACT From January 1999 to February 2007, 61 end-stage renal disease (ESRD) candidates for kidney transplantation underwent an esophagogastroduodenoscopy (EGDS) to detect Helicobacter pylori (HP). We correlated treatment for HP before transplantation and upper digestive tract hemorrhagic complications and possible recurrence of peptic disease posttransplantation. Summary Background : The majority of reflux patients have non-erosive reflux disease. Aim : To evaluate the influence of Helicobacter pylori on oesophageal acid exposure in patients with both non-erosive and erosive reflux disease and in a group of controls. The pattern and distribution of chronic gastritis were also assessed. Gastrin: an acid-releasing, proliferative and immunomodulatory peptide?

Gastrin release is affected by gastric inflammatory conditions. Antral G cells respond to inflammatory mediators by increasing gastrin secretion. Accumulating experimental evidence suggests that gastrin exerts immunomodulatory and proinflammatory effects. Gastrin could be a contributing factor to these pathologies, which may constitute a new justification for pharmacological blockade of gastrin action.

Duodenogastrischer Reflux nach Vagotomie und Pyloroplastik

Bei 27 Patienten mit selektiver oder trunkularer Vagotomie und Pyloroplastik wurde gepruft, ob ein duodenogastrischer Reflux vorliegt. Die Untersuchung erfolgte rontgenologisch durch Instillation von verdunntem Bariumbrei in das Duodenum und durch Nachweis von i.v. appliziertem Bromsulphalein im Magensaft. Es konnte bei 20 von 23 Patienten rontgenologisch und bei 18 von 27 Patienten durch den Bromsulphalein-Nachweis ein duodenogastrischer Reflux festgestellt werden. In Zusammenhang mit Reflux standen postoperative Beschwerden wie Vollegefuhl, Brechreiz und Oberbauchschmerzen, sowie der Nachweis morphologischer Veranderungen der Magenschleimhaut im Sinne von Gastritiden verschiedener Stadien.

Resolution of Ménétrier’s Disease after<i> Helicobacter pylori</i> Eradicating Therapy

Some cases of Ménétrier’s disease associated with <i>Helicobacter pylori</i> (HP) have recently been reported in the literature. We report here the case of a 51-year-old man with a diagnosis of Ménétrier’s disease who had previously been unsuccessfully treated with H<sub>2</sub> antagonists. A subsequent demonstration of HP infection led us to treat the patient with an eradicating therapy which prompted complete regression of clinical symptoms, resolution of the gastric endoscopic picture, and absence of HP on gastric histology.
