Treatment of chronic hepatitis C in hemodialysis patients

Hepatitis C virus (HCV) infection is especially problematic in patients with end-stage renal disease (ESRD) who are undergoing hemodialysis. Rates of HCV infection are higher among hemodialysis patients than in the general population, and several routes of transmission are thought to stem from the dialysis unit. Management of chronic hepatitis C is also more complicated in hemodialysis patients because of altered pharmacokinetics and a predisposition for drug-related toxicity, particularly ribavirin-induced anemia. Single Intravenous Dose Kinetics and Accumulation of Atenolol in Patients with Impaired Renal Function and on Hemodialysis

The concentration of atenolol in plasma and urine was determined following an intravenous (i.v.) dose given to 17 hypertensive patients with a glomerular filtration rate (GFR) between 5 and 105 ml/min and in 4 patients on hemodialysis. In patients with normal renal function the mean half life of elimination was calculated to be 6.8 h. This value increased to a mean of 50.1 h in patients with a GFR below 10 ml/min. In patients on hemodialysis the half life of elimination was about 4 h. Use of neural networks for dosage individualisation of erythropoietin in patients with secondary anemia to chronic renal failure.

The external administration of recombinant human erythropoietin is the chosen treatment for those patients with secondary anemia due to chronic renal failure undergoing periodic hemodialysis. The goal is to carry out an individualised prediction of the erythropoietin dosage to be administered. It is justified because of the high cost of this medication, its secondary effects and the phenomenon of potential resistance which some individuals suffer. One hundred and ten patients were included in this study and several factors were collected in order to develop the neural models. Since the results obtained were excellent, an easy-to-use decision-aid computer application was implemented.

Theoretical approach and clinical application of kinetic modelling in dialysis.

Using kinetic modelling for shaping profile dialysis, we present a promising approach to improve the cardiovascular stability of patients during dialysis treatment. In order to obtain an insight into the physiological mechanisms of increased stability, a model considering alterations of electrolytes and water distribution and of acid-base status was developed. This algorithm was used for the evaluation of 114 dialysis sessions, which were performed with highly individualised profiles.
