The history of medical records is thousand-year-long, with earlier roots in ancient civilizations. Until the 19th century, medical records mainly served educational purposes, later assuming other roles such as in insurance or legal procedures. This article comprehensively describes and reviews the development of medical records from ancient to modern times in Europe and North America, reflecting alterations and adaptations compliant with the mental and technological capabilities of a given period. Medication adherence is a significant problem in public health. Prescription-level pharmacy databases have great potential for monitoring actual drug adherence patterns at the healthcare system level. Adaptation and Refinement of Validated Risk Screening Tools in the Spanish Healthcare System

Abstract The aim of this study is to develop a tool to screen elders at risk of hospital readmissions adapted to Spanish target population. The development of this new tool is based on the validated instrument The Community Assessment Risk Screen – CARS –, which is being applied in a sample of 1.000 older patients. The adaptation and refinement of CARS is being carried out through the compilation of new potential variables from primary care clinicians to predict patients at risk of readmissions. The introduction of this new instrument adapted to Spanish target population will provide a potential tool to identify prematurel y patients at risk of hospital readmissions.

Objective: Patients with chronic, untreatable diseases have emotional needs that are often unaddressed by conventional care services within the healthcare systems. The situation of patients with idiopathic pulmonary fibrosis (IPF) is paradigmatic as there are few, if any, studies focusing on psychological interventions for these patients. This chapter describes a study that identifies and quantifies emotional distress, quality of life, spirituality, and key dimensions of health-related well-being after brief psychotherapy treatments at an outpatient clinic.
