Infective endocarditis: review of 36 cases

2009

In a retrospective study of cases of infective endocarditis (IE) observed in adult patients, the data of patients hospitalized for definite IE in the Cardiosurgery Unit of ARNAS-Civico in Palermo (Italy) from March 2003 to September 2006 were analysed. All cases were classified according to the modified Duke criteria. In all, 36 immunocompetent patients with "definite" IE were included (20 males and 16 females with a median age of 54 years). The aortic valve (23/36, 64%) was the most commonly involved, followed by the mitral (19/36, 52.7%) and tricuspid valve (4/36, 11%). In 10 patients (27.7%), a double localization was observed. Blood culture yielded a positive result in 15 cases. Staphyl…

Redundant and synergistic information transfer in cardiovascular and cardiorespiratory variability

2015

In the framework of information dynamics, new tools are emerging which allow one to quantify how the information provided by two source processes about a target process results from the contribution of each source and from the interaction between the sources. We present the first implementation of these tools in the assessment of short-term cardiovascular and cardiorespiratory variability, by introducing two strategies for the decomposition of the information transferred to heart period (HP) variability from systolic arterial pressure (SAP) and respiration flow (RF) variability. Several measures based on the notion of transfer entropy (TE) are defined to quantify joint, individual and redun…

Instantaneous transfer entropy for the study of cardio-respiratory dynamics

2015

Measures of transfer entropy have been proposed to quantify the directional coupling and strength between two complex physiological variables. Particular attention has been given to nonlinear interactions within cardiovascular and respiratory dynamics as influenced by the autonomic nervous system. However, standard transfer entropy estimates have shown major limitations in dealing with issues concerning stochastic system modeling, limited observations in time, and the assumption of stationarity of the considered physiological variables. Moreover, standard estimates are unable to track time-varying changes in nonlinear coupling with high resolution in time. Here, we propose a novel definitio…

Instantaneous Transfer Entropy for the Study of Cardiovascular and Cardio-Respiratory Nonstationary Dynamics

2017

Objective: Measures of transfer entropy (TE) quantify the direction and strength of coupling between two complex systems. Standard approaches assume stationarity of the observations, and therefore are unable to track time-varying changes in nonlinear information transfer with high temporal resolution. In this study, we aim to define and validate novel instantaneous measures of TE to provide an improved assessment of complex nonstationary cardiorespiratory interactions. Methods: We here propose a novel instantaneous point-process TE (ipTE) and validate its assessment as applied to cardiovascular and cardiorespiratory dynamics. In particular, heartbeat and respiratory dynamics are characteriz…

Lag-specific transfer entropy as a tool to assess cardiovascular and cardiorespiratory information transfer

2014

In the study of interacting physiological systems, model-free tools for time series analysis are fundamental to provide a proper description of how the coupling among systems arises from the multiple involved regulatory mechanisms. This study presents an approach which evaluates direction, magnitude, and exact timing of the information transfer between two time series belonging to a multivariate dataset. The approach performs a decomposition of the well-known transfer entropy (TE) which achieves 1) identifying, according to a lag-specific information-theoretic formulation of the concept of Granger causality, the set of time lags associated with significant information transfer, and 2) assig…

Information transfer in QT-RR dynamics: Application to QT-correction

2018

ABSTRACTThe relation between the electrical properties of the heart and the beating rate is essential for the heart functioning. This relation is central when calculating the “corrected QT interval” — an important measure of the risk of potentially lethal arrhythmias. We use the transfer entropy method from information theory to quantitatively study the mutual dynamics of the ventricular action potential duration (the QT interval) and the length of the beat-to-beat (RR) interval. We show that for healthy individuals there is a strong asymmetry in the information transfer: the information flow from RR to QT dominates over the opposite flow (from QT to RR), i.e. QT depends on RR to a larger e…

Positive inotropic response to 5-HT in human atrial but not in ventricular heart muscle

1992

The effects of 5-hydroxytryptamine (5-HT) on force of contraction (FC), action potential (AP) and calcium current (ICa) were studied in human right atrial and left ventricular heart muscle. 5-HT exerted a concentration-dependent increase in FC in multicellular atrial preparations; the EC50 was approximately 3 x 10(-7) mol/l. Maximal increases in FC (252 +/- 58% of control values; mean +/- SEM, n = 6) were obtained at 5-HT 10(-5) mol/l. At this concentration, ICa was increased four- to sevenfold in enzymatically isolated atrial myocytes. In contrast, ventricular preparations did not respond to 5-HT; FC, AP and ICa remained unaffected. In the same preparations, FC was increased by isoprenalin…

Combining physiological measures in the detection of concealed information.

2008

Meta-analytic research has confirmed that skin conductance response (SCR) measures have high validity for the detection of concealed information. Furthermore, cumulating research has provided evidence for the validity of two other autonomic measures: Heart rate (HR) and Respiration Line Length (RLL). In the present report, we compared SCR detection efficiency with HR and RLL, and investigated whether HR and RLL provide incremental validity to electrodermal responses. Analyses were based on data from 7 different samples covering 275 guilty and 53 innocent examinees. Results revealed that the area under the ROC curve was significantly higher for SCR than for HR and RLL. A weighted combination…

Effects of Acute Stress on Decision Making under Ambiguous and Risky Conditions in Healthy Young Men.

2016

AbstractAcute stress and decision making (DM) interact in life – although little is known about the role of ambiguity and risk in this interaction. The aim of this study is to clarify the effect of acute stress on DM under various conditions. Thirty-one young healthy men were randomly distributed into two groups: experimental and control. DM processes were evaluated before and after an experimental session. For the experimental group, the session consisted of an acute stress battery; and the protocol was similar for the control group but the instructions were designed to minimize acute stress. Cardiovascular variables were continuously recorded 30 minutes before the DM tasks and during the …

Multislice multiecho T2* cardiovascular magnetic resonance for detection of the heterogeneous distribution of myocardial iron overload.

2006

Purpose To assess the tissue iron concentration of the left ventricle (LV) using a multislice, multiecho T2* MR technique and a segmental analysis. Materials and Methods T2* multiecho MRI was performed in 53 thalassemia major patients. Three short-axis views of the LV were obtained and analyzed with custom-written software. The myocardium was automatically segmented into 12 segments. The T2* value on each segment as well as the global T2* value were calculated. Cine dynamic images were also obtained to evaluate biventricular function parameters by quantitative analysis. Results For the T2* global value, the coefficient of variation (CoV) for intra-/interobserver and interstudy reproducibili…
