Einfluss von Typ-II-Endoleaks auf die Größenänderung und Hämodynamik experimenteller Aortenaneurysmen nach endovaskulärer Therapie

2003

PURPOSE To evaluate the aneurysm volume and the intra-aneurysmatic pressure and maximal pressure pulse (dp/dtmax) in completely excluded aneurysms and cases with endoleaks. MATERIALS AND METHODS In 36 mongrel dogs, experimental autologous aneurysms were treated with stent-grafts. All aortic side branches were ligated in 18 cases (group I) but were preserved in group II (n = 18). Aneurysm volumes were calculated from CT scans before and after intervention, and from follow-up CT scans at 1 week, 6 weeks and 6 months. Finally, for hemodynamic measurements, manometer-tipped catheters were introduced into the excluded aneurysm sac (group I and II), selectively in endoleaks (group II), and intral…

Clinical Syndromes, Pathogenesis, and Differential Diagnosis

1991

The temporal sequence of signs and symptoms in patients with cerebral ischemia provides important information for the analysis of underlying pathophysiologic mechanisms and in the search for a major hemodynamic or embolic cause. The signs reported and symptoms assessed are useful for localization of the ischemic region of the brain and identification of the affected vascular territories. Even in the case of a typical clinical picture the clinical findings alone are often insufficient for unequivocal anatomic and pathologic identification, however important they may be in the choice of diagnostic and therapeutic measures. In the first few hours after cerebral ischemia, determining the progno…

ESRA19-0275 The efficacy and safety of ultrasound-guided superficial cervical plexus block (SCPB) in patient undergoing thyroid surgery: a case report

2019

Background and aims The SCPB provides effective anesthesia and analgesia for the head and neck region. CPBs can be performed more safely and accurately under ultrasound guidance, which is used to easily identify various important landmarks. This case report was carried out to assess the efficacy of SCPB in reducing the intra and postoperative use of opioids/analgesics to control pain due to thyroidectomy. Methods Case report: 45-year-old woman, ASA II, underwent left hemithyroidectomy under general anesthesia (sevoflorane 2% and remifentanil TCI 0.7–1 ng/ml). Ultrasound-guided left superficial cervical plexus block (SCPB) was performed using levobupivacaine 10 ml 0.25%. Pain score was measu…

Resting myocardial blood flow quantification using contrast-enhanced magnetic resonance imaging in the presence of stenosis: A computational fluid dy…

2015

Purpose: The extent to which atherosclerotic plaques affect contrast agent (CA) transport in the coronary arteries and, hence, quantification of myocardial blood flow (MBF) using magnetic resonance imaging (MRI) is unclear. The purpose of this work was to evaluate the influence of plaque induced stenosis both on CA transport and on the accuracy of MBF quantification. Methods: Computational fluid dynamics simulations in a high-detailed realistic vascular model were employed to investigate CA bolus transport in the coronary arteries. The impact of atherosclerosis was analyzed by inserting various medium- to high-grade stenoses in the vascular model. The influence of stenosis morphology was ex…

Transkranielle Dopplersonographie bei Kopf-Hals-Tumoren*

1991

Transcranial Doppler ultrasound (TCD) recording is a non-invasive diagnostic procedure for the evaluation of the cerebral collateral flow in patients, in whom therapeutic ligation or resection of the common and/or internal carotid artery is planned. Patients are first examined under resting conditions, and then under manual compression of the ipsilateral carotid artery. Since January 1989, 31 ENT and neurosurgical patients have been examined. In all patients an immediate decrease in flow velocity in the middle cerebral artery (mca) of about 25% to 90% was recorded. In 42.8% of the patients the mca flow velocity reached 90% or more of its value under normal conditions within a short period. …

Effects of Ciprostene on Restenosis Rate during Therapeutic Transluminal Coronary Angioplasty

1992

Ciprostene, a chemically stable prostacyclin analog was studied for its effects on restenosis in patients with coronary artery disease undergoing therapeutic percutaneous transluminal coronary angioplasty (PTCA). In a double-blind, randomized trial 32 patients were randomized to receive either ciprostene or the respective placebo. The infusion started intracoronarily at a rate of 40 ng/kg/min 20 min before introduction of the balloon catheter into the coronary artery. Thereafter infusion was continued intravenously for 36 hours at a rate of 120 ng/kg/min and a tapering off period until 48 hours. The quantitative analyses of the degree of coronary artery stenoses on the angiographic films be…

Druckentlastung im Aneurysmasack nach endovaskulärer Therapie von Aortenaneurysmen*

2000

PURPOSE Stent grafting of aortic aneurysms should result in relief of pressure within the excluded aneurysmal sac, however confirming data are not available. This study evaluates the intra-extraluminal pressure translation and translation of maximum pressure increase (dp/dtmax) into the excluded aneurysmal sac after endovascular treatment of experimental aortic aneurysms. MATERIALS AND METHODS Experimental autologous aneurysms were created surgically using a patch from the sheath of the rectus abdominis muscle in 12 mongrel dogs. After 12 weeks reconvalescence, endovascular treatment was performed viafemoral access using dacron-covered nitinol stents. Spiral CT and angiography were performe…

What have we learnt 50 years after the first Fontan procedure?

2020

The Fontan procedure is often the only definitive palliative surgical option for patients with a variety of complex CHD sharing in common, a single, dominant ventricle. In recent decades, imaging and therapeutic improvement have played a crucial role in those patients in whom many complications can hamper their life. After 50 years from the first procedure, heart transplantation remains the only definitive treatment for those with a failing Fontan circulation.
