In clinical practice, the clinician is challenged with symptoms and/or signs at times apparently insoluble by diagnostic and/ or therapeutic means. We propose that in these cases, we have to use an EBM approach in which evidence may be looked up in every available clinical report and bibliographic databases are used for searching that evidence. Anterior fusion for cervical spondylosis

Anterior fusion was performed on 138 patients as treatment for degenerative changes of the cervical vertebral column. The results were checked after up to 11 years in 122 patients and were found to be good in 55%, fair in 3895% and poor in 6.5%. There were complications of phonetic paralysis in one case, Horner syndrome in two cases, 2 wound infections, and complaints about the iliac crest in 3 patients. Kyphosis at the fused segment occurred 26.1% of cases, the average angle being 15.3 degrees, but it did not influence the clinical results. Mortality was 2%. In fr. I R.3 (Afranius’ Abducta) we can recognise an imagery connected to the rhetorical-philosophical Stoic tradition. Indeed, the adjective senticosus introduces the metaphor of thorns, a stylistic device associated with the Stoic sermo. This interpretation seems to be confirmed by Apuleius’ use of this adjective in Florida 18, which contains the famous ‘sophism of Protagoras’.
Informal laws and state law in the horn of Africa

This essay examines the relationship existing between informal law and state laws in the four countries of the Horn of Africa from an historical and comparative perspective. Such relation is considered from the time when the four countries became independent, however reference is also made from time to time to the colonial background when necessary to justify a particular situation. The analysis casts also an eye to the relation existing between religious laws and state laws, as religious laws represent an essential component in the interaction between state laws and the other normative orders present in these four countries. Synthesis of 2-Prenylated Alkoxylated Benzopyrans by Horner–Wadsworth–Emmons Olefination with PPARα/γ Agonist Activity

[Image: see text] We have synthesized series of 2-prenylated benzopyrans as analogues of the natural polycerasoidol, a dual PPARα/γ agonist with anti-inflammatory effects. The prenylated side chain consists of five or nine carbons with an α-alkoxy-α,β-unsaturated ester moiety. Prenylation was introduced via the Grignard reaction, followed by Johnson–Claisen rearrangement, and the α-alkoxy-α,β-unsaturated ester moiety was introduced by the Horner–Wadsworth–Emmons reaction. Synthetic derivatives showed high efficacy to activate both hPPARα and hPPARγ as dual PPARα/γ agonists. These prenylated benzopyrans emerge as lead compounds potentially useful for preventing cardiometabolic diseases.
