Perineal wound complications after abdomino-perineal rectal resection are frequent and clinically relevant for their impact on the length of hospitalization, costs, patients' quality of life and oncologic results. In this article we analyze the variables of anxiety and depression in two different situations: a combat situation and taking an exam. We used the Hospital Anxiety and Depression Scale questionnaire with a sample of 621 subjects, divided in four groups, two groups of soldiers, one of them in situation of combat (SBH) and the other one on the military service (CMS). The other two groups were composed of male students (MS) and female students (FS) that were going to take an exam. Six operational definitions of the concept of major depression were submitted to empirical evaluation in 600 psychiatric inpatients. Special attention was given to the comparison of major depression in DSM-III-R and ICD-10. The data base created by a polydiagnostic interview revealed relevant classificatory differences between the six definitions under study. Unlike the descriptive approach of DSM-IV and ICD-10, sophisticated psychoanalytical models have been proposed for many years explaining the development of physical symptoms without definable medical cause. Based on a review of the psychoanalytic models of somatization and conversion, this article explores difficulties in the medical and psychotherapeutic treatments of somatoform disorders.
