Endoscope-assisted keyhole surgery for aneurysms of the anterior circulation and the basilar apex
2000
In the treatment of cerebral aneurysms, preoperative surgical planning and intraoperative strategies for microsurgical approaches are important issues. Within the context of minimally invasive neurosurgery, so-called keyhole approaches, especially those with intraoperative endoscopic assistance, have gained increasing interest, but their application for cerebral aneurysm surgery is a matter of ongoing controversy. The main aspects of keyhole microsurgery, with and without endoscopic assistance, for cerebral aneurysms are explained. To present the keyhole concept, which is based on the preoperative analysis and intraoperative use of pre-existing anatomic windows, this article concentrates on…
Chimioradiothérapie des cancers de l’œsophage : quelles aires ganglionnaires faut-il irradier ?
2014
Esophageal cancer has a high likelihood of distant lymphatic spread even at an early stage. Radiotherapy plays a major role in the management of localized or locally-advanced esophageal cancer with a regional or distant lymph node involvement. Radiotherapy can sterilize micrometastatic nodes and cancer cells in transit in the peri-esophageal fat that are not removed by surgery. After preoperative chemoradiotherapy followed by monobloc esophagectomy including lymph node dissection above and below the diaphragm, the locoregional failure rate was around 3% in the Chemoradiotherapy for Esophageal Cancer followed by Surgery Study Group (CROSS) trial. This is significantly lower than that observe…
1-Year Outcomes of FFRCT-Guided Care in Patients With Suspected Coronary Disease
2016
Abstract Background Coronary computed tomographic angiography (CTA) plus estimation of fractional flow reserve using CTA (FFR CT ) safely and effectively guides initial care over 90 days in patients with stable chest pain. Longer-term outcomes are unknown. Objectives The study sought to determine the 1-year clinical, economic, and quality-of-life (QOL) outcomes of using FFR CT instead of usual care. Methods Consecutive patients with stable, new onset chest pain were managed by either usual testing (n = 287) or CTA (n = 297) with selective FFR CT (submitted in 201, analyzed in 177); 581 of 584 (99.5%) completed 1-year follow-up. Endpoints were adjudicated major adverse cardiac events (MACE) …

Einfluss von Typ-II-Endoleaks auf die Größenänderung und Hämodynamik experimenteller Aortenaneurysmen nach endovaskulärer Therapie
2003
PURPOSE To evaluate the aneurysm volume and the intra-aneurysmatic pressure and maximal pressure pulse (dp/dtmax) in completely excluded aneurysms and cases with endoleaks. MATERIALS AND METHODS In 36 mongrel dogs, experimental autologous aneurysms were treated with stent-grafts. All aortic side branches were ligated in 18 cases (group I) but were preserved in group II (n = 18). Aneurysm volumes were calculated from CT scans before and after intervention, and from follow-up CT scans at 1 week, 6 weeks and 6 months. Finally, for hemodynamic measurements, manometer-tipped catheters were introduced into the excluded aneurysm sac (group I and II), selectively in endoleaks (group II), and intral…
Towards clinical application of RayStretch for heterogeneity corrections in LDR permanent 125-I prostate brachytherapy
2017
Purpose RayStretch is a simple algorithm proposed for heterogeneity corrections in low-dose–rate brachytherapy. It is built on top of TG-43 consensus data, and it has been validated with Monte Carlo (MC) simulations. In this study, we take a real clinical prostate implant with 71 125I seeds as reference and we apply RayStretch to analyze its performance in worst-case scenarios. Methods and Materials To do so, we design two cases where large calcifications are located in the prostate lobules. RayStretch resilience under various calcification density values is also explored. Comparisons against MC calculations are performed. Results Dose–volume histogram–related parameters like prostate D90, …
La sinoviortesis radioisotópica en la artritis reumatoide. Estudio de 108 casos
2000
There are several therapeutic alternatives in the local treatment of rheumatoid arthritis (RA): intraarticular corticosteroids, chemical synoviorthesis, surgical synovectomy and synoviorthesis with radioisotopes. We present the results of an observational study on radioisotopic synoviorthesis carried out in the Valencian health care area from January 1989 to May 1997 which included 108 synoviortheses performed in 51 patients. This study aimed to assess the efficacy and safety of radiosynoviorthesis in the treatment of RA which had not responded to conventional treatment. Good and moderate results were obtained in 76.2% of the cases, there being significant differences in the efficacy of syn…
Die Sonoanatomie des Spinalkanales der LWS
2008
UNLABELLED Results of an Experimental Study and Report of Intraoperative Application: AIM Intraoperative sonography has been used in neurosurgery (especially in tumors) and in traumatology (assess reduction of dorsal vertebral body fragments). This study was performed to determine the value of sonography in detecting vertebral disc tissue. RESULTS We examined 6 specimens of the Lumbar spine using the extended flavectomy approach. We found that sono-anatomy was reproducible and that the assessable area was large enough to make sonography useful for detection of sequesters. Dislocated vertebral disc tissue could be differentiated clearly from original disc tissue and from other intraspinal st…
Snapshot Spectral Imaging
2018
This chapter describes an application of the spline-based wavelet frames to the spectral imaging. It presents a method that enables to convert a regular digital camera into a snapshot spectral imager by equipping the camera with a dispersive diffuser and with a compressed sensing-based algorithm for digital processing. The method relies on the assumption that typical images can be sparsely represented in the frame domain. The solution is found from the constrained \(l_{1}\) minimization of a functional by Bregman iterations. Results of optical experiments are reported. The chapter is based on the paper (Golub et al., Appl. Opt. 55, 432–443, (2016), [11]).
Third-harmonic generation in optical microfibers
2012
We explain the relatively easy, wideband, THG conversion that we observe experimentally in silica glass microfibers by the tapering geometry. As a challenging perspective, we compare THG effective efficiencies in silica and tellurite glasses.
