Differential impact of syncope on the prognosis of patients with acute pulmonary embolism: a systematic review and meta-analysis

2018

Aims Controversial reports exist in the literature regarding the prognostic role and therapeutic implications of syncope in patients with acute pulmonary embolism (PE). We conducted a systematic review and meta-analysis to investigate the association between syncope and short-term adverse outcomes, taking into account the presence or absence of haemodynamic compromise at acute PE presentation. Methods and results The literature search identified 1664 studies, 29 of which were included for a total of 21 956 patients with PE (n = 3706 with syncope). Syncope was associated with higher prevalence of haemodynamic instability [odds ratio (OR) 3.50; 95% confidence interval (CI) 2.67-4.58], as well…

Der Metastasendurchmesser ist entscheidend für das lokale Behandlungsergebnis nach Kryotherapie colorectaler Lebermetastasen

1998

In a retrospective analysis we aimed to assess the incidence of local recurrence at the cryosite as well as possible prognostic indicators for the disease-free interval at the cryosite following hepatic cryotherapy for non-resectable colorectal metastases. At a median follow-up of 22 months, 66 of 85 patients had developed tumor recurrence, involving local recurrence at the cryosite in 28 patients, with cryoablated metastases of >3 cm being associated with shorter disease-free intervals at the cryosite. Improvements in probe placement and monitoring of the freezing process are required to allow successful treatment of large liver metastases.

Direct antiviral agents in hepatitis C virus related liver disease: Don’t count the chickens before they’re hatched

2021

Since molecules with direct-acting antiviral (DAA) became available, the landscape of the treatment of hepatitis C virus (HCV) infection has completely changed. The new drugs are extremely effective in eradicating infection, and treatment is very well tolerated with a duration of 8-12 wk. This review aims to report the outstanding clinical benefits of DAA and to highlight their critical disadvantages, identifying some clinically relevant hot topics. First, do the rates of virological response remain as high when patients with more advanced cirrhosis are considered? Large studies have shown slightly lower but still satisfactory rates of response in these patients. Nevertheless, modified sche…

Incidence of HSV and HPV with azathioprine

2009

IBd Incidence of HSV and HPV with azathioprine Mario Cottone and Sara Renna severe infections are an established risk of immunosuppressive therapy; however, the risk of opportunistic infections in patients with IBd who receive immunosuppressive therapy has so far only been studied retrospectively. the increased incidence of herpes flares and development or worsening of viral warts in patients with IBd who receive azathioprine has now been demonstrated for the first time in a prospective study. the rate of opportunistic infections in patients with iBD is dependent on the patient’s nutritional status, degree of innate immune system activity, whether the patient has undergone surgery and their…

Nonalcoholic Fatty Liver Disease Increases the Risk of Anxiety and Depression.

2020

Nonalcoholic fatty liver disease (NAFLD), depression, and anxiety disorders are frequent diseases, and data on mutual influence are inconsistent. The aim of this study was to explore the incidence of depression and anxiety in a large primary care cohort in Germany and to study the impact of NAFLD over a 10‐year time frame. Patients with NAFLD diagnosed between 2010 and 2015 were matched to a cohort without NAFLD controlling for age, sex, physician, index year, and Charlson comorbidity index. The primary outcome of the study was the incidence of depression, anxiety, and first prescription of antidepressant drugs. We compared 19,871 patients with NAFLD to 19,871 matched controls. Within 10 ye…

Postoperatives kognitives Defizit bei alten Patienten

2008

Postoperative congnitive dysfunction occurs in geriatric patients after major operations and influences morbidity and mortality of these patients. Clinical observations and neuropsychological tests can be used to diagnose cognitive disorders after an operation. Until today no specific medical treatment is known for the therapy of cognitive dysfunctions after an operation and, therefore, in high risk patients perioperative preventive measures is the only way to decrease the incidence and extend of this disease.

The effects of muscle strength and power training on mobility among older hip fracture patients

2008

The incidence of hip fractures is growing in all Western societies. The mobility of hip fracture patients does not return to the pre-fracture level even 2 years after fracture. One reason for mobility limitation may be the persistent muscle weakness on the fractured leg. The purpose of this randomized controlled study was to examine whether 12-week muscle strength and power training twice a week has an effect on mobility in 60–85-year-old hip fracture patients. Forty-three persons were randomly assigned to an intervention (n=23) and a control (n=20) group. The intervention comprised 12-week supervised intensive progressive strength–power training twice per week. All the measurers were blind…

Systematic review and meta-analysis of cardiovascular risk in rheumatological disease: Symptomatic and non-symptomatic events in rheumatoid arthritis…

2022

Abstract Although each autoimmune disease is associated with specific tissue or organ damage, rheumatic diseases share a pro-inflammatory pattern that might increase cardiovascular risk. Retrospective and prospective studies on patients affected by systemic lupus erythematosus (SLE) and rheumatoid arthritis (RA) highlighted the concept of “accelerated atherosclerosis”. Therefore, the purpose of this systematic review and meta-analysis is the assessment of symptomatic or asymptomatic cardiovascular events among patients with rheumatic diseases as RA and SLE. The literature research obtained all manuscripts published in the English language between 2015 and 2019 for a total of 2355 manuscript…

Safety and tolerability of omalizumab

2009

Summary Background Omalizumab (Xolair®) is a recombinant humanized monoclonal anti-IgE antibody with proven efficacy in patients with moderate-to-severe and severe persistent allergic (IgE-mediated) asthma. Objective To review clinical study data to assess the safety profile of omalizumab. Methods We analysed the safety of omalizumab using data from completed clinical studies (up to 1 year) involving more than 7500 patients with asthma, rhinitis or related conditions and up to 4 years in one study of patients with severe allergic asthma, as well as post-marketing safety data. Analysis focuses on the risk of immune-system effects, hypersensitivity reactions, malignant neoplasia, parasitic in…

Conspiracy of silence and emotional distress in patients diagnosed with glioblastoma multiforme

2012
