A matter of fat: insulin resistance and oxidative stress
2012
Background Obesity is linked to insulin resistance (IR), which can lead to type 2 diabetes mellitus. Oxidative stress present in early obesity may favor the progression to comorbid conditions. Objective To examine the relationship between oxidative stress biomarkers and the severity of IR in a group of obese children. Methods Forty obese children with a body mass index (BMI) Z-score ≥ 2 were divided into two groups using the median obtained for the homeostasis model assessment of IR (HOMA-IR). Anthropometric parameters (including body fat composition by bioelectrical impedance) and biochemical parameters were assessed. The following biomarkers of oxidative stress were measured: malondialdeh…
Real-Time Breath Analysis in Type 2 Diabetes Patients During Cognitive Effort
2013
The understanding the functional expression of exhaled volatile organic compounds (VOCs) has gradually expanded from the initial identification of breath pathological markers to direct expression of physiological activity. In the present study we investigated the potential application of breath analysis in real-time monitoring of type 2 diabetes mellitus (T2DM) patients versus control subjects while performing a cognitive task. T2DM is associated with cognitive impairment and neural deficits, because of insulin resistance and high expression of insulin receptors in the hippocampus. We set out to seek the evidence for mutual associations among breath exhale, on the one side, and T2DM and cog…
The role of elevated growth hormone on the increased atherosclerosis in patients with acromegaly.

Die rechnergestützte Diabeteseinstellung
1985
Blood glucose self-monitoring is essential to stabilize diabetes on a normoglycemic level. But often patients do not find the correct insulin adaptation. Therefore, we developed a computerized system which adapts insulin doses using a control matrix. During a first in-patient period we had a remarkable blood glucose normalization and stabilization.
Low risk of severe hypoglycaemia in patients with type 2 diabetes mellitus starting insulin therapy with premixed insulin analogues BID in outpatient…
2012
Summary Aims: The choice of insulin at initiation in type 2 diabetes remains controversial. The aim of this study was to assess the occurrence of self-reported severe hypoglycaemia associated with premixed insulin analogues in routine clinical care. Methods: A 12-month, prospective, observational, multicentre study in patients starting a commonly prescribed premixed insulin analogue (either insulin lispro 25/75 or biphasic insulin aspart 30/70, twice daily) after suboptimal glycaemic control on oral antidiabetic agents. Treatment decisions were made solely in the course of usual practice. Results: Study follow-up was completed by 991 (85.5%) of the 1150 patients enrolled. At baseline, me…
Los niveles plasmáticos de IL-18 se relacionan con la insulinemia y están modulados por polimorfismos del gen de IL-18
2015
INTRODUCTION Atherosclerosis is an inflammatory chronic disease influenced by multiple factors. Different prospective studies have shown that plasmatic levels of inflammatory markers were related to atherosclerosis and cardiovascular disease. OBJECTIVE To evaluate whether plasmatic levels of interleukin 18 (IL-18) are modulated by SNPs (single nucleotide polymorphisms) of the IL 18 gene and its possible association with insulin levels and other cardiovascular risk factors. METHODS 746 individuals were studied for a period of two years by opportunistic selection in the metropolitan area of Valencia. Parameters of lipid and glucose metabolism were analyzed by standard methodology. IL-18 was m…
Diabetes and Dementia
2015
Persons with type 2 diabetes mellitus (DM2) have an increased incidence of cognitive decline and dementia. An increased cortical and subcortical atrophy has been found after controlling for vascular disease and inadequate cerebral circulation. A possible role of insulin resistance and hyperinsulinemia has been suggested to mediate the link between DM2 and Alzheimer’s Disease (AD). Altered insulin signaling may contribute to AD biochemical and histopathological lesions. Both hyperglycemia and hypoglycemia may contribute to cognitive decline in DM2. Recurrent symptomatic and asymptomatic hypoglycemic episodes have been suggested to cause subclinical brain damage, and permanent cognitive impai…
Physical exercise and sarcopenia in older people: position paper of the Italian Society of Orthopaedics and Medicine (OrtoMed)
2015
Sarcopenia is the age-associated loss of skeletal muscle mass and function. It is a major clinical problem for older people and research in understanding of pathogenesis, clinical consequences, management, and socioeconomic burden of this condition is growing exponentially. The causes of sarcopenia are multifactorial, including inflammation, insulin resistance, changing endocrine function, chronic diseases, nutritional deficiencies and low levels of physical activity. Operational definition of sarcopenia combines assessment of muscle mass, muscle strength and physical performance. The diagnosis of sarcopenia should be based on having a low appendicular fat free mass in combination with low …
Influence of Metabolic Control on Thromboxane Biosynthesis and Plasma Plasminogen Activator Inhibitor Type-1 in Non-insulin-dependent Diabetes mellit…
1996
SummaryWe have previously shown that tight metabolic control by insulin therapy reduced thromboxane-dependent platelet activation in noninsulin-dependent diabetes mellitus (NIDDM) patients. The present study was undertaken to determine whether a similar effect could be obtained without switching diabetics in secondary failure to insulin treatment. For this purpose, we gave strict diet and exercise advise program and adjusted on a weekly basis the oral antidiabetic therapy (glipizide) that 26 patients with NIDDM had been given over the previous months.Basal measurements of urinary ll-dehydro-TXB2 and PAI-1 confirmed previous findings of enhanced levels of these parameters in NIDDM patients w…
