New intramedullary locking nail for olecranon fracture fixation--an in vitro biomechanical comparison with tension band wiring.

The aim of this study was to determine the difference in displacement of a newly designed intramedullary olecranon fracture fixation device compared with multifilament tension band wiring after 4 cycles and 300 cycles of dynamic continuous loading.In eight pairs of fresh-frozen cadaver ulnae, oblique olecranon fractures were created and stabilized using either newly designed intramedullary olecranon nail or multifilament tension band wiring. The specimens were then subjected to continuous dynamic loading (from 25 N to 200 N) using matched pairs of cadaveric upper extremities. Abstract Background. The aim of this study was to develop a test-setup with continuous angle alteration to imitate joint motion for the evaluation of 3 different olecranon fracture fixation devices. Methods. Twenty-one fresh cadaver upper extremities underwent olecranon fracture by the means of transverse osteotomy and received 3 different fixation systems. Kuntscher nailing of femoral shaft fractures in children and adolescents.

We have reviewed 29 cases of very unstable diaphyseal fractures of the femur in children and adolescents treated by Kuntscher nailing. Follow up was for at least 5 years, and in most cases until after the end of growth. There were no post-operative complications, and the mean overgrowth of the femur was 7.06 mm. There was growth disturbance of the proximal femur in one case, but this did not interfere with function. We believe that Kuntscher nailing is indicated in children and adolescents with unstable high velocity fractures of the femoral shaft.

Intraosseous Fixation Compared to Plantar Plate Fixation for First Metatarsocuneiform Arthrodesis

Background: Metatarsocuneiform (MTC) fusion is a treatment option for management of hallux valgus. We compared the biomechanical characteristics of an internal fixation device with plantar plate fixation. Methods: Seven matched pairs of feet from human cadavers were used to compare the intramedullary (IM) device plus compression screw to plantar plate combined with a compression screw. Specimen constructs were loaded in a cyclic 4-point bending test. We obtained initial/final stiffness, maximum load, and number of cycles to failure. Pathogenesis and prophylaxis of circulatory reactions during total hip replacement

Circulatory reactions such as a drop in blood pressure, bradycardia, cardiac arrest, and even intraoperative death after insertion of the stem are well known events during total hip replacement. The present paper reports bone marrow intravasation after rise of intramedullary pressure in the femoral cavity during insertion of hip prostheses, demonstrated by intraoperative transesophageal echocardiography. In an animal study, the ultrasound echoes were identified as "mixed emboli" consisting of a core of bone marrow surrounded by thrombus. Effect of intramedullary gradual elongation of the shorter limb on gait patterns

Background: Seven young patients were followed 52 weeks after intramedullary lengthening for limb length discrepancy (LLD). The mean LLD before surgery was 3.0 ± 1.1 cm and 1 year after surgery at the time of the nail-removal it was 0.3 ± 0.3 cm. Methods: The plantar pressures and the ground reaction forces (GRF) were recorded simultaneously with electromyographic (EMG) activities at normal and fast walking speeds. Results: Bilateral comparison indicated that the uncorrected LLD resulted in asymmetrical gait patterns. Humeral nailing revisited.

Unreamed interlocked humeral nailing for stabilisation of acute humeral fractures was introduced a decade ago. Antegrade and retrograde nail insertion are equally popular. The role of nailing as opposed to plating of humeral fractures is the subject of continuous debate. Between 1997 and 2005, 99 acute fractures of the humeral shaft were treated operatively with the unreamed humeral nail (UHN, Synthes) in our Level I Trauma Centre. The mean age of the patients was 63 years. Only eight patients (8.1%) were polytraumatised, nine patients had an open fracture (9.1%), five had a primary radial nerve palsy (5.1%).
