Prävention des früh-postoperativen IOD-Anstiegs nach Phakoemulsifikation

2001

Purpose. To compare the ocular hypotensive effect of different antiglaucomatous eye drops. Patients and methods. This double-blind, randomized study included 119 eyes of 119 patients without other ocular pathology undergoing standardized (one surgeon, Healon) small incision cataract surgery with foldable intraocular lens implantation. The patients were assigned to one of five groups: group 1:0.25% timolol in Gelrite (extended efficacy, n=23); group 2: 2% dorzolamide (n=24); group 3: combination of 0.5% timolol plus 2% dorzolamide (n=22); group 4: brimonidine (n=26); group 5: gentamicin (control, n=24). Intraocular pressure (IOP) was measured preoperatively and at 3±1,6±1,9±1,24±3, and 48±3 h…

Argon laser suture lysis following glaucoma filtering surgery – A short introduction to the procedure

2010

Abstract Background Trabeculectomy is the most commonly performed glaucoma filtering surgery. The success of the outcome of an operation to reduce the intraocular pressure (IOP) is dependent on postoperative wound modulation by antimetabolite therapy and by flow regulation of the filtering bleb, which can be achieved by argon laser suture lysis. Methods and case report Postoperative IOP controls, examination of bleb formation and bleb massage can lead to the decision to perform up to four argon laser suture lyses in order to reduce IOP and enhance aqueous humor flow and bleb formation. The procedure is described here in a case report. Conclusion After glaucoma filtering surgery, argon laser…

Current Status of Epibulbar Anti-glaucoma Drainage Devices in Glaucoma Surgery

2012

SUMMARY Background: The term “glaucoma” covers a hetero geneous group of progressive optic neuropathies that are accompanied by characteristic visual-field defects. Primary open-angle glaucoma, the most common type, progresses insidiously and causes blindness if untreated. All current forms of treatment aim at lowering the intraocular pressure (IOP) in patients whose IOP is elevated. The implantation of anti-glaucoma drainage systems is one of the available options for surgical treatment. Methods: This review is based on pertinent literature retrieved by a selective search, including glaucoma treatment guidelines from Germany and abroad. Results: A paradigm shift is currently underway regar…

Dietary polyunsaturated fatty acids, intraocular pressure, and glaucoma

2014

Abstract Glaucoma is a progressive optic neuropathy and is the second leading cause of blindness worldwide. Estimations predict that nearly 80 million people will be affected by the disease in 2020. Increased intraocular pressure (IOP) has been recognized as a major risk factor for developing glaucoma. Although IOP-lowering agents have been shown to be effective for decreasing the progression of glaucoma, extensive research has been done to identify modifiable lifestyle factors involved in the pathogenesis of this disease. Among these factors, dietary lipids and particularly dietary polyunsaturated fatty acids (PUFAs) represent a promising approach. The aim of this chapter is to summarize t…

Latanoprost-induced iris darkening: a morphometric study of human peripheral iridectomies.

2003

Abstract Purpose. This microscopic study was undertaken to compare the melanocytes of peripheral iridectomy specimens from two eyes that had latanoprost-induced iris darkening (LIID) with iridectomies taken from the fellow untreated eyes. Methods. The two patients in this study were the ones who underwent LIID in the latanoprost treated eye from a series of 17 patients requiring bilateral trabeculectomy. The first trabeculectomy procedure provided a control peripheral iridectomy for each patient, whereas the second eye was treated with once daily 50 μg ml−1 latanoprost drops for 6 months. The four peripheral iridectomy specimens from the two LIID patients were subjected to quantitative morp…

Implantation of a custom intraocular lens with a 30-diopter torus for the correction of high astigmatism after penetrating keratoplasty.

2003

Abstract We present a 62-year-old woman who had implantation of an intraocular lens (IOL) with a torus of 30.0 diopters (D) in an eye after penetrating keratoplasty. The patient presented with cataract and a best corrected visual acuity of 20/400. Penetrating keratoplasty was performed in 1975 because of decompensated keratoconus. With a clear but inferiorly decentered corneal graft, visual acuity was limited because of keratoplasty-related high corneal astigmatism of 22.5@156 that could not be successfully treated with spectacles or contact lenses. A custom toric IOL with –30.0 D of cylindrical power was implanted in the capsular bag via a sclerocorneal tunnel incision. Additionally, a sph…

Optical quality of intraocular lenses after several years in the human eye

2013

The optical quality of explanted intraocular lenses (IOLs) was analysed to verify if their quality is maintained after having been in the human eye for several years. Twenty-five explanted IOLs were studied, 10 because of intraocular luxation and 15 from corneal donors. The dioptric power and modulation transfer function (MTF) were determined to discover the image quality of the IOLs. The spectral transmission was also established to ascertain whether the filter against ultraviolet (UV) radiation was still effective. The results show that the MTF of implanted IOLs is maintained and therefore so is the image quality they provide. Only decay in the MTF of some of the explanted IOLs was observ…

Optical analysis to predict outcomes after implantation of a double intraocular lens magnification device.

2007

Purpose To analyze the effect of different optical parameters of the eye on the outcomes of implantation of a double intraocular lens (IOL) magnification device in patients with low vision. Setting Fundacion Oftalmologica del Mediterraneo, Valencia, Spain. Methods The eye's refraction and magnification before and after surgery were evaluated using a theoretical eye model based on paraxial optics approximation. Four parameters on which refraction and magnification are dependent were evaluated: anterior chamber depth (ACD), axial length (AL), mean keratometry (Km), and the distance between the 2 implanted IOLs. The 4 variables were analyzed separately in the first stage. Next, different combi…

Visual And Refractive Outcomes In Hyperopic Pseudophakic Patients Implanted With A Trifocal Intraocular Lens

2019

Purpose To assess visual and refractive results after bilateral implantation of a trifocal intraocular lens (IOL) in patients with hyperopia. Methods In a retrospective nonrandomized study, 196 eyes of 98 patients had bilateral implantation of a trifocal IOL. The Barrett Universal II formula was used for IOL power calculation. Eyes were divided into two groups for their analysis: low-moderate, with IOL power ranging from 22 to 26 D, and high, with IOL power ranging from 25 to 34 D. Refractive error was used to assess predictability, and corrected distance visual acuity (CDVA) and uncorrected distance visual acuity values were used to assess efficacy and safety of the surgery. Results Six mo…
