Recombinant, activated factor VII for surgery in factor VII deficiency: a prospective evaluation - the surgical STER
2010
Excessive bleeding represents a major complication of surgical interventions and its control is especially relevant in patients with Congenital Bleeding Disorders (CBD). In factor VII (FVII) deficiency, scanty data on surgery is available to guide treatment strategies. The STER (Seven Treatment Evaluation Registry) is a multi-centre, prospective, observational, web-based study protocol providing the frame for a structured and detailed data collection. Inhibitor occurrence was checked in a centralized fashion. Forty-one surgical operations (24 'major' and 17 'minor') were performed in 34 subjects with a carefully characterized FVII deficiency under the coverage of recombinant activated Facto…
Can we compare haemophilia carriers with clotting factor deficiency to male patients with mild haemophilia?
2020
Introduction Certain haemophilia carriers demonstrate an increased bleeding tendency, mainly related to clotting factor deficiency. No study has so far formally compared the bleeding phenotype of women and girls with mild FVIII or FIX deficiency and associated management with that of male patients affected by mild haemophilia A and B. Material and methods We retrospectively evaluated 44 women and girls with mild FVIII or FIX deficiency (FVIII or FIX 0.05-0.5 IU/mL) and 77 male patients with mild haemophilia A or B and compared them with respect to clotting factor level, age at and trigger for diagnosis, as well as treatment modalities. Results After excluding gender-related haemorrhagic sym…
Electronic Design for a Bleeding Detector to be Used in Intraoperative Radiotherapy Applications
2021
An electronic design for a bleeding detector used with mobile linac during intraoperative radiotherapy (IORT) is presented. It consists on a readout circuit based on a De Sauty Bridge and a wireless power supply using a supercapacitor. The goal is the measurement of the height of fluid present in the applicator in order to avoid a misadministration dose during the irradiation process. A minimum measurement height of 5 mm is required by clinic to be confident with the selected radiation dose. The measurement circuit uses a capacitive sensor which detects variations lower than 0.1 pF. The readout circuit converts dynamic capacitance variation from the bleeding detector into an analog voltage …
Relationship between upper gastrointestinal bleeding and non steroidal anti-inflammatory
1997
Abstract The authors evaluated the relationship between drug intake and upper gastrointestinal (GI) bleeding. The endoscopic files of the previous 2 years were reviewed and the incidence and age and sex distribution recorded. GI bleeding is indication for a high percentage of lower endoscopies and a low percentage of upper endoscopies. On the other hand, although rarer upper GI bleeding is more severe and frequently related to drug ingestion. About 50% of cases showed gastric erosions secondary to drug intake. A relation between gastric bleeding and paracetamol is considered, as is the possibility of preventing secondary severe bleeding by pharmacologic gastric protection in children with r…
Prophylaxis In Congenital Factor VII Deficiency, Indications, Efficacy and Safety: Results of the STER
2010
Abstract Abstract 665 Introduction Prophylaxis is considered a difficult endeavour in FVII deficiency, especially because of the very short FVII zymogen and FVIIa half-lives, mainly in childhood. The Seven Treatment Evaluation Registry (STER, www.targetseven.org) is a prospective observational, multi-centre, web-based registry concerned with the evaluation of treatments for spontaneous bleeding episodes, surgery and prophylaxis in patients with FVII deficiency. As regards prophylaxis, STER provides the frame for a structured and detailed data capture aiming at: a) identifying patients in whom prophylaxis is advisable, b) defining clinical settings in which prophylaxis is necessary and c) de…
Prevention of Upper Gastrointestinal Bleeding in Cirrhotic Patients
1987
The prevalence of varices in patients with cirrhosis is stated to be about 50% and the risk of variceal bleeding 40% with mortality ranging from 30% to 60%. Differences may be due to patient selection and diagnostic criteria. The death risk of first bleeding seems to be higher than that of subsequent episodes (Christensen et al. 1981; D’Amico et al. 1986), indicating that the first bleeding episode causes a selection.
Giant uterus in a patient with Klippel-Trenaunay syndrome. Report of a case
2022
Introduction: Klippel-Trenaunay syndrome is an uncommon congenital disease also called angio-osteodystrophy syndrome for its typical disorders characterized by abnormal growth of the soft tissues and bones and vascular malformations. Case report: In this report, we present a rare case of a 46-year-old nulliparous woman with Klippel-Trenaunay syndrome. She suffered from an abnormal uterine bleeding accompanied by severe anemia with need for multiple blood transfusions. At the time of admission, physical examination revealed port-wine stains and varicose veins on her lower limbs and hypertrophy of left lower extremity. We carried out an open bilateral hysteroannexectomy. Histopathology examin…
Evaluation of different transvaginal sonographic diagnostic parameters in women with postmenopausal bleeding
1998
Objective To determine whether the accuracy of transvaginal sonography to detect endometrial pathology is enhanced by assessing endometrial morphology and the regularity of the endometrial border in addition to measurement of endometrial thickness in women with postmenopausal bleeding. Design and methods A total of 159 women with post-menopausal bleeding were included in a prospective study performed at the Clinic of Obstetrics and Gynecology, University of Mainz. The results obtained on transvaginal sonographic assessment of the endometrium were compared with the histological evaluation of the operative endometrial specimen. Results A cut-off level for endometrial thickness of 5 mm had a s…
Immediate post-extraction implants placed in acute periapical infected sites with immediate prosthetic provisionalization : a 1-year prospective coho…
2020
Background Few studies have reported the outcomes of immediate placement at infected post-extraction sites. The aim of this study was to compare clinical and radiological outcomes of immediately placed implants with immediate prosthetic provisionalization in sockets with or without acute periapical pathology. Material and Methods A total of 100 patients with immediately placed implants with immediate provisionalization and 1- year of follow up were included (50 patients with acute periapical pathology and a control group of 50 patients without acute periapical pathology). Clinical parameters (bleeding on probing, buccal keratinized mucosa width, clinical recession, and probing depth) and ra…
