Optical and Visual Simulation of Standard and Modified Spherical Aberration Implantable Collamer Lens Post Myopic LASIK Surgery

To evaluate the visual quality achieved in patients undergoing standard or modified implantable Collamer lens (ICL) to correct residual myopic error after laser-assisted in situ keratomileusis (LASIK) surgery.The adaptive optics visual simulator was used to simulate LASIK surgery of moderate and high myopia with a myopic regression corrected by a standard and modified ICL. Image quality and visual performance in the peripheral visual field following photorefractive keratectomy.

ABSTRACT PURPOSE: A theoretical and experimental study was performed to assess the possible effects of photorefractive keratectomy (PRK) on retinal image quality and thresholds in the peripheral visual field. METHODS: Simple optical calculations suggest that although the quality of the retinal image at the fovea of the postoperative PRK eye may be comparable to that in an emmetropic eye, images in the peripheral field may be markedly worse, since peripheral ray bundles may pass partly through ablated and partly through unablated cornea, giving a simultaneous-vision bifocal effect. Pain-evoked blink reflex

The electrically evoked blink reflex (BR) consists of an ipsilateral R1 component (R1) at 11 ms and two bilateral components R2 at 33 ms and R3 at 83 ms. It is still unclear whether the R2 is mediated by activation of tactile or nociceptive afferents. For testing the nociceptive hypothesis, nociceptors of the supraorbital nerve were selectively activated by infrared laser stimuli in 10 subjects. Only painful laser stimuli evoked a bilateral early polyphasic BR response (LR2) at 71 ms. Stimulation of infraorbital and mental nerve dermatomes was equally effective. Nociceptive masseter inhibitory reflexes evoked by laser radiant heat and electrical stimuli

Electrical stimulation of the mental nerve evokes two suppression periods SP1 and SP2 in masseter muscle activity bilaterally. In order to investigate a possible nociceptive origin of the suppression periods, we compared the reflex responses evoked by electrical stimulation and by selective activation of nociceptors in hairy skin using painful infrared laser stimuli. The SP was elicited during more than 90% maximal voluntary contraction. Thresholds for detection, pain, and SP in the mental nerve area were determined by the method of limits. Abolished laser-evoked potentials and normal blink reflex in midlateral medullary infarction.

We investigated two patients presenting with the rare finding of almost isolated hemianalgesia with a sensory level on the contralateral side sparing the face. Clinical findings, electrophysiological studies (absent laser-evoked pain-related somatosensory potentials, normal electrically evoked somatosensory potentials, magnetically evoked potentials, and blink reflexes), and magnetic resonance imaging showed the ventrolateral medullar tegmentum containing the spinothalamic tract to be affected by lacunar infarction. The blink reflex R2 component was unimpaired in both patients.

Clinical evaluation criteria for the assessment of impaired pain sensitivity by thulium-laser evoked potentials

Abstract Objectives : Cortical potentials evoked by carbon dioxide laser pulses have been applied in clinical practice to study nociceptive pathways for several years. In this study, we evaluate the properties of an infrared laser (thulium-YAG) with a penetration depth in the skin that matches the intracutaneous depth of nociceptors. Methods : Temperature measurements and modelling showed that the thulium laser generates painful intracutaneous temperatures with less surface heating than the carbon dioxide laser and with no side effects (up to 600 mJ pulse energy). Photodynamic therapy of nodular basal cell carcinoma with multifiber contact light delivery.

To overcome the limited treatment depth of superficial photodynamic therapy we investigate interstitial light delivery. In the present work the treatment light was delivered using a system in which three or six clear-cut fibers were placed in direct contact with the tumor area. This placement was thought to represent a step toward general purpose interstitial PDT. Twelve nodular basal cell carcinomas were treated employing delta-aminolevulinic acid and 635 nm laser irradiation. Negative and positive laser evoked potential (LEP) peaks (N2*, P2**) were simultaneously recorded from the primary somatosensory (SI), parasylvian, and medial frontal (MF: anterior cingulate and supplementary motor area) cortical surfaces through subdural electrodes implanted for the surgical treatment of intractable epilepsy. Influence of Refraction on Tonometric Readings After Photorefractive Keratectomy and Laser Assisted In Situ Keratomileusis

PURPOSE To determine the decrease of Goldmann tonometry after photorefractive keratectomy (PRK) and laser assisted in situ keratomileusis (LASIK) according to refraction prior to surgery. METHODS Prospective simultaneous comparative case series of 53 myopic eyes (53 patients) that underwent PRK and 50 (50 patients) that underwent LASIK using the Summit Excimed SVS plus (Summit Technology, Inc., Walthan, MA, U.S.A.).
