Medication safety in a psychiatric hospital

Objective: We sought to assess the epidemiology of medication errors (MEs) and adverse drug events (ADEs) in a psychiatric hospital. Methods: We conducted a 6-month prospective observational study in a 172-bed academic psychiatric hospital. Errors and ADEs were found by way of chart review, staff reports and pharmacy intervention reports. Physicians rated incidents as to the presence of injury, preventability and severity of an injury. Serious MEs were nonintercepted MEs with potential for harm (near misses) and preventable ADEs. Results: We studied 1871 admissions with 19,180 patient-days. In previous eye movement research on word length effects, spatial width has been confounded with the number of letters. McDonald (2006) unconfounded these factors by rendering all words in sentences in constant spatial width. In the present study, the Arial font with proportional letter spacing was used for varying the number of letters while equating for spatial width, while the Courier font with monospaced letter spacing was used to measure the contribution of spatial width to the observed word length effect. The impact of a history of competitive sports on later smoking behaviour and occurrence of chronic pulmonary diseases is poorly known. We investigated how a history of elite level sports predicted later pulmonary disease morbidity and mortality. Gaucher disease is the most common lysosomal storage disease with a high prevalence in the Ashkenazi Jewish population but it is also present in other populations. The presence of eight mutations (1226G, 1448C, IVS2+1. 84GG, 1504T, 1604T, 1342C and 1297T) and the complete deletion of the beta-glucocerebrosidase gene was investigated in 25 unrelated non-Jewish patients with Gaucher's disease in Germany.
