Optimal savings and health spending over the life cycle

This paper investigates the relationship between saving and health spending in a two-period overlapping generations economy. Individuals work in the first period of life and live in retirement in old age. Health spending is an activity that increases quality of life and longevity. Empirical evidence shows that both health spending and saving behave as luxury goods but their behaviour differs markedly according to the level of per capita GDP. The share of saving on GDP has a concave shape with respect to per capita GDP, whereas the share of health spending on GDP increases more than proportionally with respect to per capita GDP. Treatment of clinically localized renal tumors in the elderly

There are several options for treating clinically localized renal tumors in the elderly, ranging from active surveillance to radical nephrectomy. Reduced renal function is associated with increased cardiovascular mortality, so maintaining renal function is of the utmost importance. Personalized therapy should be based on tumor location, comorbidities and general health status rather than tumor size and patient age.

Newborn infants and the moral significance of intellectual disabilities.

This article presents moral philosophical arguments regarding life-saving medical treatment that may be more available to infants without disabilities than to infants with intellectual disabilities. The ideas are that children with disabilities are a burden to their families and to society and that a happy life may not be attainable for these children and their families. I argue that human well-being is not based merely on individual characteristics, but is a result of the individual's relation to other people. Free Flaps for Anterior Chest Wall Reconstruction

Reconstruction of full-thickness chest wall defects usually entails the use of regional pedicled myocutaneous flaps as first-choice option. As defect complexity increases and local options are not available, microsurgical reconstruction is required to transfer reliable and well-vascularized tissues from distant sites. Size, anatomic location, presence of dead spaces, and extent of eventual bone defect should be assessed for proper selection of the flap. Regional recipient vessels should be previously identified and checked in order to consider alternatively the need of extension of distant recipient vessels through arteriovenous (AV) loops or vein graft interposition.

Die transpupillare Thermotherapie (TTT) wird erfolgreich zur Behandlung kleiner Aderhautmelanome am hinteren Augenpol eingesetzt [9, 13, 14]. Wir berichten uber die erfolgreiche Behandlung von Aderhautmetastasen mittels TTT unter intravenoser Gabe von Indozyaningrun (ICG). Bei 3 Patienten mit systemisch metastasierenden Karzinomen (Osophagus-, Bronchial- und Mamma-Ca) wurden Aderhautfiliae in 4 Augen mit jeweils einer Sitzung TTT behandelt. Verwendet wurde ein Diodenlaser (810 nm) und Spotgrose 2–3 mm. Eye fluke-induced cataract formation in fish: quantitative analysis using an ophthalmological microscope

We examined the parasite-induced cataract formation in rainbow trout (Oncorhynchus mykiss) using slit-lamp microscopy to determine the relationship between cataract intensity and number of Diplostomum spathaceum parasites that were established in the lens. Cataract intensity increased significantly with parasite burden, but was also affected by the pattern of exposure to the parasite cercariae. The slit-lamp methodology proved useful in scoring the cataracts since it provides a 3-dimensional view into the lens and gives an actual picture of the location and intensity of the cataracts, which allows detailed investigations of mechanisms underlying cataract formation in various fish species. Dilation of the ductus venosus by stent implantation increases placental blood perfusion in fetal sheep.

The reduction of resistance to flow in the ductus venous (DV) and a decrease of blood supply to the liver serve for the survival of the fetus during hypoxia. The present study investigated the influence of the increased diameter of the DV on placental blood perfusion.In 15 ewes with twin pregnancies at gestational ages of 117 +/- 4 days, a stent (4 or 5 mm) was placed into the DV of 1 twin (DV(stent) group) under ultrasound guidance. Blood flow rates in the umbilical vein (UV) and DV of both fetuses were measured using Doppler ultrasound. Residual coronary stenosis after thrombolysis with rt-PA or streptokinase: acute results and 3 weeks follow-up

Ninety-one patients with acute myocardial infarction were assigned to intravenous treatment with streptokinase or rt-PA as part of the randomized trial carried out by the European Study Group for Recombinant Tissue-Type Plasminogen Activator (rt-PA). A patent coronary artery was found in 37 of 45 (82%) patients treated with rt-PA and in 27 of 46 (59%) patients treated with streptokinase 75-90 minutes after start of infusion. Patients were subsequently anticoagulated with heparin or dicoumarol up to a repeat angiography 3 weeks after the infarction.
