Humeral nailing revisited.

2008

Unreamed interlocked humeral nailing for stabilisation of acute humeral fractures was introduced a decade ago. Antegrade and retrograde nail insertion are equally popular. The role of nailing as opposed to plating of humeral fractures is the subject of continuous debate. Between 1997 and 2005, 99 acute fractures of the humeral shaft were treated operatively with the unreamed humeral nail (UHN, Synthes) in our Level I Trauma Centre. The mean age of the patients was 63 years. Only eight patients (8.1%) were polytraumatised, nine patients had an open fracture (9.1%), five had a primary radial nerve palsy (5.1%). There were 54 antegrade and 45 retrograde nailings. The procedures were performed …

Sex differences in renal damage induced in the mouse by Amanita virosa.

1977

Summary The sensitivity of male and female mice to Amanita virosa was compared. Dried, homogenizedmushroom was given orally by stomach tubing at doses of 100, 200, 400 and 800 mg dried mushroom/kg body weight. Both in males and in females, the kidneys were the only organs showing macroscopical changes. The dose of 100 mg/kg caused renal damage in females, whereas in males the first signs of kidney damage were seen at the dose of 400 mg/kg. The renal lesions observed in the males were located in the cortex, while in the females they were limited to the outer stripe of the outer medullary zone. Testectomy diminished the nephrotoxicity of A. virosa in male mice and caused changes in the locali…

Medullary thyroid carcinoma in a 2-month-old male with multiple endocrine neoplasia 2B and symptoms of pseudo-Hirschsprung disease: a case report

2007

A 5-week-old male patient was seen for symptoms suggestive of Hirschsprung disease (abdominal distension, failure to thrive, and explosive defecation). Rectum biopsies revealed an intestinal ganglioneuromatosis, which is usually associated with multiple endocrine neoplasia (MEN) syndrome type 2B. The ensuing molecular genetic analysis revealed a M918T mutation of the RET protooncogene, which is associated with early-onset medullary thyroid carcinoma (MTC). Therefore, total thyroidectomy and central lymphadenectomy were performed at the age of 9 weeks. Histology showed a medullary microcarcinoma. This report of MTC occurrence within the first weeks of life underlines the importance of early …

Thickness sensitivity of ultrasound velocity in long bone phantoms

2004

One approach to bone disease diagnosis such as osteoporosis is to measure the velocity of ultrasound propagating axially along long bones. In this study, the variation in velocity as a function of radial position was assessed using two polyvinyl chloride (PVC) bone phantoms with cross-sectional geometry similar to the human tibia but differing in medullary cavity diameter. Two ultrasonometers were used: these were a commercial device operating at a relatively high frequency (HF) of 1.25 MHz and a prototype low frequency (LF) device operating at approximately 200 kHz. The LF measurements showed a larger variation with radial position, with changes in velocity of up to 20% occurring around th…

Nachweis von Markembolien wÄhrend Oberschenkelmarknagelungen mittels transösophagealer Echokardiographie

1989

Pathogenesis of circulatory reactions triggered by nervous reflexes during the implantation of bone cements.

1983

Circulatory and respiratory reactions during the implantation of joint endoprostheses have been observed for years. The components of the bone cements used for anchoring (methyl methacrylate monomer) or the outpour of bone marrow substances from the affected bone marrow cavity are thought to be chiefly responsible for these effects. However, our previous investigations demonstrated unambiguously that reactions of this type can be attributed to direct nervous-reflex mechanisms triggered by the pressure rise in the medullary canal, and that, in addition, bone marrow embolism plays a part in serious circulatory incidents. The investigations on which the present paper is based have confirmed th…

“Weekly docetaxel and gemcitabine as first line treatment for metastatic breast cancer: results of a multicenter phase II study”

2004

<i>Objectives:</i> We conducted a multicenter phase II study to evaluate the clinical efficacy, toxicity, and dose intensity of a new weekly schedule of docetaxel and gemcitabine as first-line treatment of metastatic breast cancer patients. <i>Methods:</i> We enrolled 58 patients, 52% of whom had received a previous anthracycline-containing chemotherapy. The treatment schedule was: docetaxel 35 mg/m2 and gemcitabine 800 mg/m2 i.v. on days 1, 8, 15 every 28 days. <i>Results:</i> All patients were assessable for toxicity and 56 for efficacy. Overall response rate was 64.3% with 16.1% of complete responses and 48…

Dynamische Analyse von Osteosynthesen am Olecranon: ein in-vitro Vergleich zweier Osteosynthesesysteme / Dynamic analysis of olecranon osteosyntheses…

2008

INTRODUCTION The aim of the present study was to develop a test setup with continuous angle alteration to imitate elbow joint motion for the mechanical evaluation of tension band wiring and a newly designed intramedullary nail. MATERIALS AND METHODS The servo-pneumatical test stand worked with a rotational angle-adjusted and a linear force-adjusted engine. The fracture model was dynamically tested under cyclic loading imitating elbow joint motion. In total, 14 fresh cadaver upper extremities underwent olecranon fracture by means of transverse osteotomy and were assigned to two groups: tension band wiring and intramedullary nailing. There was a continuous angle alteration between 0 and 1000 …

A Clinical Comparison between the Gamma® and the Claufitt® Nails in Unstable Proximal Femoral Fractures

2007

Purpose To assess the differences that exist between two intramedullary osteosynthesis systems for the treatment of unstable proximal femoral fractures in an attempt to find out whether the use of an antirotation screw improves the final result. Materials and methods A retrospective study was carried out to analyze the plcement and clinical evolution of both intramedullary nailing systems (Gamma ® y Claufitt ® ). The patients’ medium-term functional status was evaluated and benchmarked against the characteristics of the population served by our University Hospital. A final database was obtained made up of 43 cases of Claufitt ® that met the criteria of our study. A functional assessment sys…

Intramedullary nailing vs. palmar locked plating for unstable dorsally comminuted distal radius fractures: A biomechanical study

2010

Abstract Background The purpose of this study was to compare the stability of a 2.4 mm palmar locking compression plate and a new intramedullary nail-plate-hybrid Targon DR for dorsally comminuted distal radius fractures. Methods An extraarticular 10 mm dorsally open wedge osteotomy was created in 8 pairs of fresh frozen human radii to simulate an AO–A3-fracture. The fractures were stabilized using one of the fixation methods. The specimens were loaded axially with 200 N and dorsal-excentrically with 80 N. 2000 cycles of dynamic loading and axial loading-to-failure were performed. Findings Axial loading revealed that intramedullary osteosynthesis (Targon DR: 369 N/mm) was significantly ( p …
