Morphometric Examination of the Dilated Rat Ureter

1987

Dilatations of the ureter are known in rats with diabetes insipidus as well as after the administration of diuretics (Wladimiroff 1975). Nevertheless, similarities to congenital deformities, such as the primary megaureter, have not been established to date. It has not yet been determined whether in the fetus increased diuresis can lead to a functional overstrain of the transport capacity of the ureter and thus to mutations in the sense of a congenital megaurter. In addition to the direct influence of increased diuresis, teratogenic effects of the administered diuretics during pregnancy are possible. Furthermore, it should be determined whether the dilatation can be classified as an adaptive…

End Cutaneous Ureterostomy for the Management of Severe Hydronephrosis

2007

Purpose: Although rarely indicated, surgical treatment of severe megaureter can pose a formidable technical challenge, especially in the small infant. We present our experience and outcomes with end cutaneous ureterostomy as a temporizing adjunct to future ureteral reimplantation. Materials and Methods: We performed a retrospective cohort study of patients who underwent end cutaneous ureterostomy between 1993 and 2005. Patient demographics, surgical details and outcomes were recorded. Results: A total of 29 patients (22 males, 7 females) underwent diversion of 34 renal units. Primary megaureter was diagnosed in 15 patients (17 renal units). Secondary megaureter was found in 10 patients (12 …

Influence of induced intrauterine diuresis on the upper urinary tract

1993

There are many possible etiologies of sonographically detected dilatation of the renal pelvis and the ureter during pregnancy. The observation that fetal ureteral dilatation occurs with congenital diabetes insipidus has led to the suspicion of at least a modifying influence resulting from intrauterine diuresis. To increase fetal diuresis experimentally, Wistar rats were treated with 100 mg chlorthalidon and 40 mg furosemide per kg bodyweight per day. The animal experiment was performed with four groups. Group I was treated during the entire course of pregnancy. Group II during the first half and Group III during the second half. Group C was the untreated control group. For morphometric inve…

Antibiotic prophylaxis in antenatal nonrefluxing hydronephrosis, megaureter and ureterocele

2012

Observation is a conservative management option in infants with nonrefluxing hydronephrosis, primary nonrefluxing megaureter and ureterocele diagnosed postnatally following antenatal detection of hydronephrosis. Antibiotic prophylaxis might be a sensible regimen under these circumstances to prevent UTI in this population who are potentially at increased risk. However, studies examining the efficacy of prophylactic antibiotics are sparse in this setting. For each condition, prophylactic policies seem extremely variable, and UTI rates vary widely with comparable rates reported between patients followed on and off antibiotics. Overall, antibiotic prophylaxis seems unnecessary in patients with …

Primary megaureter in adults.

1984

The terminology of megaureters, their histological findings and etiology vary greatly in the literature. Primary obstructed megaureters must be clearly differentiated from megaureters caused by infravesical obstruction. The characterization and therapeutical consequences of the two types also differ. In a retrospective study, 43 adult patients with 54 primary obstructed megaureters (male:female = 30:13) were studied. This diagnosis excluded infravesical obstruction, reflux and neurogenic disorders. The main symptoms were flank pain, infection and hematuria. An operation was performed in 25 patients; 29 were treated conservatively. The criteria for surgery are discussed in relation to sympto…

Double-J stent insertion across vesicoureteral junction--is it a valuable initial approach in neonates and infants with severe primary nonrefluxing m…

2006

Abstract Objectives To evaluate the role of double- J stent insertion in perinatally detected primary nonrefluxing megaureters as a method to temporize treatment in patients with impaired renal function or to prevent function loss in patients treated expectantly, but deemed at high risk of deterioration. Methods Two neonates and 8 infants with a ureter greater than 10 mm and an obstructive excretion pattern, including 3 cases with renal function less than 40%, were selected to undergo double- J stent insertion for a 6-month period. Patients underwent surgery if the ureter redilated and the excretion pattern was obstructive at reassessment 3 months after stent removal. Results Stents were pl…

Angiotensin type 2 receptor is important in the normal development of the ureter

1999

In humans, the actions of angiotensin II are transduced through the AT1 and AT2 receptors which have recently been implicated in renal organogenesis. Polymorphisms in the human angiotensin II receptor genes have been linked to cardiovascular and nephrological disorders. In this study we evaluated 35 patients with either primary obstructive megaureter or posterior urethral valves. Each was genotyped for the A1166 AT1 polymorphism and the recently described A-1332G AT2 transition. The incidence of these genetic variants was also evaluated in normal controls without any ultrasonographic urological abnormalities. Similar to our previous findings in congenital urological abnormalities, the AT1 r…

Angiotensin II, type 2 receptor in the development of vesico-ureteric reflux

2001

Objective To investigate if mutation of the angiotensin II (Ang II) receptors AT2 is involved in primary vesico-ureteric reflux (VUR) in humans. Patients and methods Genetic polymorphisms in the AT1 and AT2 receptors was evaluated in 23 patients having the most common congenital urological abnormality, namely primary congenital VUR. The occurrence of the A1166C transition in the AT1 receptor gene and the A-1332G transition in the AT2 receptor gene were evaluated and compared with the incidence in normal controls with no urological abnormalities. Result The distribution of the AT1 receptor genotypes was no different between patients with VUR and healthy controls. Furthermore, 10 of 23 (44%) …
