Anterior dislocation in a total knee arthroplasty: A case report and literature review.

Dislocations of a total knee arthroplasty (TKA) are an uncommon injury and only few cases of anterior dislocations have been reported. We report a rare case of anterior dislocation of a 10-year-old posterior stabilized total knee arthroplasty in a 74-year-old woman. The patient was successfully treated by close reduction of the dislocation followed by immobilization in full extension with a long leg cast. Because of a high risk of neurovascular complications, a high index of suspicion for vascular injury must be maintained. For this reason, a proper diagnosis is required and immediate reduction is recommended.

With conservative or operative management, we have to take care of destroyed articular surfaces and traumatised soft tissues. Between 1974 and 1984 16 patients with rheumatoid arthritis, who had elbow synovectomy and excision of the radial head, were revised with an average follow-up of 5 years after operation. Resection of the radial head was performed on 16 elbows, in 3 elbows the ulnar nerve was transposed. The indication for the operation were pain, swelling, warmth and radiological destruction. The parameters used, to assess efficacy of the operation were range of motion, local finding and patient satisfaction. Synovectomy results were good especially in respect of relief of pain and range of motion. The good results justify elbow synovectomy even in the late stages of disease.

Osteoporosis is a noncommunicable disease with increasing incidence at a global level. The burden of the disease particularly concerns women. A drastic and rapid decline in the circulating levels of estrogens, a key regulator of bone metabolism, is responsible for the increase in bone loss after menopause. Cells with a role in bone metabolism, osteoblasts, osteoclasts and osteocytes, have estrogen receptors. The fall in estrogens increases the differentiation of osteoclasts from progenitors in the bone marrow. A significant expansion in the resorption process follows.

Medialization of the center of rotation in reverse shoulder arthroplasty (RSA) results in greater excursion of the proximal humerus especially in adduction leading to a mechanical conflict against the neck of the scapula, thus creating a specific complication: scapular notching, in up to 90 % of RSA. We describe our third generation of RSA in Dijon, the Duocentric prosthesis. This prosthesis provides a new proposition to prevent the notching problem with an inferior overhang integrated to the glenoid baseplate. They account for approximately 2–3 % of all fractures and for 17–30 % of fractures around the elbow. In younger patients, there is a predominance among males. Celecoxib for recurrent sclerouveitis after syphilitic panuveitis. A case report

Purpose To report a case of recurrent ocular inflammation after optimal therapy of bilateral syphilitic panuveitis responding to oral celecoxib. Methods A case report was conducted. Results A 76-year-old man presented with painful blurry vision in both eyes. Ocular examination disclosed bilateral panuveitis. Serological testing confirmed blood and cerebrospinal fluid syphilitic involvement. After 2 weeks of intravenous penicillin therapy, recurrent episodic sclerouveitis was observed. Conclusion Ocular inflammation after healing of infectious uveitis is a rare ophthalmic sequela. In an immunocompetent patient, either re-infection or immune uveitis should be evoked.
