Our Definition of Propeller Flaps and Their Classification

AbstractThe term propeller flap was introduced for the first time by Hyakusoku to define an island flap, based on a subcutaneous pedicle hub, that was rotated 90 degrees to correct scar contractures due to burns. With the popularization of perforator flaps, the propeller movement was applied for the first time to a skin island vascularized only by an isolated perforator, and the terms propeller and perforator flap were used together. Thereafter, the surgical technique of propeller flaps evolved and new applications developed. With the “Tokyo consensus,” we proposed a definition and a classification schema for propeller flaps. Background: Fabry disease is an X-linked glycosphingolipid storage disorder due to a deficiency of the enzyme alpha-galactosidase A. Accumulation of substrate results in a progressive and life-threatening multisystemic disease. Early clinical manifestations include pain and gastrointestinal symptoms. Sensorineural hearing loss and vertigo are well-recognized features of the disorder, occurring in approximately 50% of adults with Fabry disease. Abstract Electrode migration is a challenge, even with adequate anchoring techniques, due to the high mechanical stress on components of occipital nerve stimulation (ONS) for headache disorders. When a lead displacement of an ONS implant is diagnosed, there are currently different approaches described for its management. The future: critical knowledge about anti-itch therapy.

:  Itch is an extremely frequent and enervating symptom of many diseases. Current anti-itch therapy, which is based almost exclusively on an “immunocentric” viewpoint, is often unsatisfactory. Recent studies show that this symptom is in fact the result of a complex interplay among skin, nervous system, endocrine system, and immune system. This explains the frequent failure of therapeutic strategies focused only on a single factor and suggests the usefulness of a polypharmacologic symptomatic treatment, designed on a case-by-case basis as a result of a multidisciplinary approach. We discuss the perspectives of anti-itch therapy in light of the new pathogenetic and pharmacologic acquisitions.

Validation of Knee KL-classifying Deep Neural Network with Finnish Patient Data

Osteoarthritis (OA) is the most common form of joint disease in the world. The diagnosis of OA is currently made by human experts and suffers from subjectivity, but recently new promising detection algorithms have been developed. We validated the current state-of-the-art KL-classifying neural network model for knee OA using knee X-rays taken from postmenopausal women suffering from knee pain attributable to OA. The performance of the model on the clinical data was considerably lower compared to the previous results on population-based test data. ABSTRACTWhile the adult subependymal zone (SEZ) harbors pools of distinct neural stem cells that generate different types of GABAergic interneurons, a small progenitor population in the dorsal SEZ expresses Neurog2 and gives rise to glutamatergic neurons. Here we investigated whether SEZ progenitors can be programmed towards glutamatergic neurogenesis through forced expression of Neurog2. The position of the artificial lens has an important influence on refractive power calculation. We compared the position of the crystalline lens with that of the artificial lens after cataract surgery by means of Scheimpflug photography. A difference in position of approximately 0.8 mm in the anterior direction could be determined.
