Lower Urinary Tract Symptoms: What's New in Medical Treatment?

2018

Abstract Context Pharmacological treatment is a cornerstone in the management of patients with lower urinary tract symptoms (LUTS). Objective To review emerging evidence in the medical treatment of LUTS. Evidence acquisition An Embase/Pubmed-based literature search was conducted in December 2017, screening for randomized controlled trials (RCTs), prospective and retrospective series, animal model studies, and reviews on medical treatment of LUTS. Evidence synthesis The main medical innovation in recent years in overactive bladder (OAB) has been the approval of the first β 3 -adrenoceptor agonists (mirabegron) and intradetrusor onabotulinum toxin A, while several other drugs such as antiepil…

Fatigue, sleep, and nocturnal complaints in patients with amyotrophic lateral sclerosis

2012

Background and purpose: Fatigue is a common symptom in amyotrophic lateral sclerosis (ALS). Although sleep disturbances are a candidate factor that may interfere with fatigue in patients with ALS, the role of sleep-related abnormalities in determining fatigue in ALS is unknown. Objective: To evaluate the frequency and determinants of fatigue in a group of 91 consecutive patients with ALS, with special attention to the relationship between fatigue and sleep problems. Methods: Measures included the Fatigue Severity Scale (FSS), Pittsburgh Sleep Quality Index (PSQI), Epworth Sleepiness Scale (ESS), ALS Functional Rating ScaleRevised (ALSFRS-R), and Beck Depression Inventory (BDI). Results: The…

Pathophysiology orientated therapy of the aging bladder

2004

Aging is associated with changes of organ systems including skin, bone metabolism and the muscoloskeletal, endocrine, cardiovascular, and genitourinary systems . Concerning age-related changes in lower urinary tract function, the incidence of nocturia and general neurological or vascular diseases with impact on detrusor function are comparable for men and women. However, there are also sex related differences in lower urinary tract symptoms [LUTS) such as bladder outlet obstruction in men (due to agerelated prostate growth) and gonadal insufficiency, multiparity, cystocele or enterocele in women. In addition to the common occurrence of LUTS in elderly people, comorbidity increases with age,…

Do overactive bladder symptoms and their treatment-associated changes exhibit a normal distribution? Implications for analysis and reporting

2020

Aims: To explore the use of means vs medians (assuming or not the presence of normal distribution) in studies reporting overactive bladder syndrome symptoms and to test for normal distribution of basal values and treatment-associated changes thereof in two large noninterventional studies. Methods: Systematic review of all original studies reporting on at least one overactive bladder syndrome symptom published in four leading urology journals in 2016 to 2017. Testing of the normal distribution of urgency, incontinence, frequency, and nocturia in two large noninterventional studies (n = 1335 and 745). Results: Among 48 eligible articles, 86% reported means (assuming a normal distribution), 6%…

Sleep–wake problems in patients with amyotrophic lateral sclerosis: implications for patient management

2012

SUMMARY Sleep–wake problems are frequent, although unrecognized, complications of amyotrophic lateral sclerosis (ALS). Sleep disorders such as insomnia, sleep-disordered breathing and restless legs syndrome have all been reported in patients with ALS, despite the limited number of studies and the small populations investigated so far. Sleep disturbances gradually worsen with disease progression, suggesting a relationship between the severity of disease and the neurodegenerative process. However, poor sleep can also be a consequence of several disturbances such as anxiety, depression, pain, choking, sialorrhea, fasciculations, cramps, nocturia and the inability to get comfortable and move f…

OAB Evidence from the Urologist’s Perspective

2003

Abstract Overactive bladder (OAB) is the term introduced by the International Continence Society in 2002 to describe the symptoms of urgency, with or without urge incontinence, usually in the presence of frequency and nocturia. Community-based prevalence studies have shown an estimated 16% of the adult population in the US and Europe suffers the symptoms of OAB, and in many cases the symptoms can have a profound effect on quality of life. OAB can also co-exist with other disorders (e.g. depression, sleep deprivation, urinary tract and skin infections) which further increases the burden to the patient. Contemporary therapeutic algorithms, based on clinical experiences and evaluations of the …
