Blood-pressure variability in patients with obstructive sleep apnea: current perspectives
2018
Obstructive sleep apnea (OSA) is often associated with hypertension and other cardiovascular diseases. Blood pressure (BP) variability is part of the assessment of cardiovascular risk. In OSA, BP variability has been studied mainly as very short-term (beat-by-beat) and short-term (24-hour BP profile) variability. BP measured on consecutive heartbeats has been demonstrated to be highly variable, due to repeated peaks during sleep, so that an accurate assessment of nocturnal BP levels in OSA may require peculiar methodologies. In 24-hour recordings, BP frequently features a "nondipping" profile, ie, <10% fall from day to night, which may increase cardiovascular risk and occurrence of major…
Spontaneous Nocturnal Erections - Physiology and Clinical Applications. Spontane nachtliche Erektionen - Physiologie und klinische Anwendungen
2005
Spontaneous nocturnal erections in men are a well-known phenomenon whose physiology however is not yet really understood. The measurement of nocturnal erections has gained clinical relevance for the diagnosis of erectile dysfunction because it allows an objective and quantitative assessment of the erectile capacity. A decisive advantage compared to other diagnostic tests carried out during wakefulness is the exclusion of psychological factors to a great extent. Thus, an evaluation of the organic component of an erectile dysfunction is possible. Regarding the interpretation of nocturnal erection records, various factors possibly influencing the results must be considered; this is particularl…

Prescription of automatic bilevel ventilation (AutoBI) in sleep-disordered breathing: analysis according to diagnosis and occurrence of comorbidities
2017
CPAP is the first-choice treatment for obstructive sleep-disordered breathing (O-SDB), while the criteria to prescribe AutoBI are undefined. Trials of AutoBI ventilation in patients (pts) treated for O-SDB from January 2015 to October 2016 (64 out of 242 titrations) were reviewed to assess: a) the clinical characteristics of these pts, b) the reason to shift from CPAP to AutoBI, c) the compliance to prescribed treatment. AutoBI was used in cases of intolerance to high therapeutic CPAP levels (n=21) or incomplete resolution of SDB on CPAP (n=43). The Table reports the differences between CPAP or AutoBI pts CPAP SDB diagnosis was: 82% OSA, 18% OSA-COPD Overlap in the CPAP group; 39% OSA, 28% …
Predictors of Progression in Hypertensive Renal Disease in Children
2004
In hypertensive renal disease in children, several risk factors influence the development and the rate of progression of renal damage, including blood pressure levels, proteinuria, lipid disorders, and genetic differences. The impact of blood pressure on renal structures, the most important of the factors, depends not only on blood pressure levels, but also on the persistence of the blood pressure levels over time, mainly during the hours when the patient is resting or sleeping. Abnormal circadian variability is frequently observed in patients with renal damage, and nocturnal blood pressure reduction should be a major therapeutic objective to protect against a decline in renal function. Pro…
