Occlusion of two adjacent cortical veins is followed by a widespread reduction of rCBF and the occurrence of small infarcts, which become larger if spreading depression (SD) occurs. The infarct matures over time with TUNEL-positive cells seen in the penumbra up to 4 days after vein occlusion. Caspase inhibition with zVAD.fmk reduces infarct size. Here, the time course of gene expression in the penumbra is compared to that induced by SD alone.

Recent studies seem to favor a beneficial effect of hyperventilation (HV) in cases of cerebrovascular occlusion [1, 4] and brain trauma [2, 5]. On the other hand, it seems that, at least under certain circumstances, HV may exert an adverse effect upon cerebral blood flow and metabolism [3]. Since this question is not settled yet, it seems worthwhile to report briefly the present case of reversible total occlusion of the middle cerebral artery closely followed by successive rCBF studies (133Xe-gamma-clearance method), in which, during the acute phase, HV caused a latent ischemia to become manifest.

Purpose To examine the impact of intravitreal aflibercept (IVT-AFL) injection on the 25-item National Eye Institute Visual Function Questionnaire (NEI VFQ) in the COPERNICUS and GALILEO trials. Methods Patients (pts) with macular oedema secondary to central retinal vein occlusion received IVT-AFL 2 mg (IVT-AFL 2q4) or sham monthly for 24 weeks. After Week 24 (W24) in COPERNICUS all pts were eligible to receive IVT-AFL based on visual and anatomical outcomes (IVT-AFL 2q4+PRN, sham+IVT-AFL PRN).
