Oral trasmucosal fentanyl citrate for breakthrough pain treatment in cancer patients.

2012

Breakthrough cancer pain has been defined as a transitory increase in pain intensity that occurs either spontaneously or in relation to a specific predictable or unpredictable trigger, despite relatively stable and adequately controlled background pain. The availability of supplemental doses of oral opioids, in addition to the continuous analgesic medication, is the main treatment suggested to manage pain flares.Oral transmucosal fentanyl citrate (OTFC) is the first product of a new generation of delivery systems, named rapid-onset opioids (ROOs), characterized by rapidity of effect and the short duration of analgesia. Controlled studies and long-term experience have shown that OTFC is an e…

Statins and new-onset diabetes

2013

Statins are highly efficacious lipid modifying agents that reduce the risk for cardiovascular (CV) events in both primary and secondary prevention settings. However, statins affect molecular mechanisms which adversely impact on insulin sensitivity and β-cell function, thereby increasing risk for new onset diabetes mellitus (NOD). Defining the mechanisms involved is the focus of considerable current investigation. The statins reduce the risk for CV events in normoglycemic patients as well as in those with diabetes mellitus (DM) and their benefits outweigh the risk of inducing NOD. We review the clinical evidence for NOD with statin treatment, as well as the potential mechanisms involved. Our…

The deformedN=60 nucleus 41 101 Nb

1991

The β− decay of101Zr has been investigated at the fission-product separators JOSEF and LOHENGRIN. The half-life of101Zr has been determined to 2.5(1) s and a level scheme for101Nb has been established fromγ ray singles as well as X/3-γ and γ—γ coincidence measurements. Conversion coefficients for transitions in101Nb and level half-lives between 10 ps and 2 ns have been determined. Three rotational bands are identified among the low-lying levels with band heads at 0 keV, 206 keV and 208 keV. The bands are probably based on the Nilsson configurations [422 5/2+], [301 3/2−] and [303 5/2−], respectively. The deformation has been determined to βq=0.40(4) and 0.41(8) for the ground state band and…

The Proton-Neutron Interaction in Neutron-Rich A ≃ 100 Nuclei

1988

The possible factors responsible for the sudden onset of nuclear deformation in neutron-rich A≅100 nuclei are reviewed from the systematics of subshell occupation numbers for neutron and proton orbitals. The agreement between our results from a Nilsson model with existing selfconsistent HFB calculations strengthens the role of a cumulative neutron-proton interaction, but does not substantiate the conjecture concerning a selective involvement of the πg9/2-νg7/2 spin-orbit-partner orbitals in producing deformation as das suggested in earlier work.

PCR for the detection of pathogens in neonatal early onset sepsis.

2020

Background A large proportion of neonates are treated for presumed bacterial sepsis with broad spectrum antibiotics even though their blood cultures subsequently show no growth. This study aimed to investigate PCR-based methods to identify pathogens not detected by conventional culture. Methods Whole blood samples of 208 neonates with suspected early onset sepsis were tested using a panel of multiplexed bacterial PCRs targeting Streptococcus pneumoniae, Streptococcus agalactiae (GBS), Staphylococcus aureus, Streptococcus pyogenes (GAS), Enterobacteriaceae, Enterococcus faecalis, Enterococcus faecium, Ureaplasma parvum, Ureaplasma urealyticum, Mycoplasma hominis and Mycoplasma genitalium, a …

Tonnenkonkremente im Ductus choledochus bei einer 4 Jahre vergessenen, impaktierten Endoprothese

2006

We report on a 67-year-old female patient who presented in July 2005 with sudden onset of pain in the right upper abdomen. The patient had undergone cholecystectomy in 1987. Because of recurrrent complaints in the right upper abdomen, a pigtail stent was placed into the common hepatic duct in 2001. When the patients presented now, the laboratory tests including liver enzymes were within normal ranges. Endoscopic retrograde cholangiogi aphy, however, revealed a remaining 10-French, impacted double pigtail endoprothesis that was obstructed by sludge as well as multiple giant bile duct stones of 20 to 30 mm in size. The giant stones could be finally removed by the combined use of ESWL and endo…

Semantic context-processing deficit in thought-disordered schizophrenic patients: Evidence from new semantic priming paradigms

2006

Introduction. Disorders in the processing of the semantic context are now a well-established phenomenon in thought-disordered (TD) schizophrenic patients, and have been revealed especially well by studies that have made use of the experimental paradigm of lexical decision tasks coupled with semantic priming. The main question addressed by this study was the evaluation of the experimental conditions under which TD schizophrenic patients are able to deploy cognitive strategies for semantic context processing. Methods. We studied semantic priming in two double lexical decision tasks (i.e., involving the explicit processing of the prime word) using a sequential presentation of words (stimulus o…

Psychosis, symbol, affectivity 2: another perspective on the treatment of psychotic disorder

2021

This is the second of two papers concerning our study into an integrated approach to psychotic disorders conducted at the University Psychiatry Unit of Palermo's Polyclinic over approximately 15 years; this paper concentrates on the clinical phenomena. The study aimed to find the best possible treatment and to improve the prognosis of this patient group. We have explored the efficacy of a range of psycho-therapeutic (cognitive-behavioural, systemic-relational, psychodynamic, group and others), psycho-pharmaceutical, psychiatric rehabilitative and psycho-educational treatments, with a hermeneutic approach instead of a systematic one. The study's conclusions, described in the paper, are that …

Iatrogenic acute aortic dissection type A: insight from the German Registry for Acute Aortic Dissection Type A (GERAADA)†

2013

OBJECTIVES: Previous investigators have reported a grave prognosis for iatrogenic acute aortic dissection (iAADA), but such studies are limited by their small sample sizes. The purpose of the current study was to analyse the clinical characteristics, current management and surgical outcomes in a large number of iAADA patients identified through a multicentre registry. METHODS: Between July 2006 and June 2010, 50 centres participated in the German Registry for Acute Aortic Dissection Type A (GERAADA). Of the 2137 patients included, 100 (5%) had iAADA. We compared the clinical features and 30-day outcomes of patients with iatrogenic and spontaneous acute aortic dissection type A (sAADA). RESU…
