Thickness sensitivity of ultrasound velocity in long bone phantoms
2004
One approach to bone disease diagnosis such as osteoporosis is to measure the velocity of ultrasound propagating axially along long bones. In this study, the variation in velocity as a function of radial position was assessed using two polyvinyl chloride (PVC) bone phantoms with cross-sectional geometry similar to the human tibia but differing in medullary cavity diameter. Two ultrasonometers were used: these were a commercial device operating at a relatively high frequency (HF) of 1.25 MHz and a prototype low frequency (LF) device operating at approximately 200 kHz. The LF measurements showed a larger variation with radial position, with changes in velocity of up to 20% occurring around th…
The burden of previous fractures in hip fracture patients. The Break Study
2011
A positive history of fractures in older patients with hip fracture is common. We determined the risk factors associated with a positive history of fractures and the profile of care in hip fracture patients. In the Break Study, we enrolled 1249 women aged ≥60 years, seeking care for a hip fracture. Baseline information included age, body mass index, lifestyle (smoking habit, alcohol consumption), patient's history of fracture after the age of 50 years, family history of fragility fracture and health status (presence of comorbidity, use of specific drugs, pre-fracture walking ability, type of fracture, time to surgery, type of surgery, osteoporosis treatment). In the multivariable model age,…

Osteonecrosis of the jaw induced by receptor activator of nuclear factor-kappa B ligand (Denosumab) - Review.
2015
Background Denosumab, an anti-resorptive agent, IgG2 monoclonal antibody for human Receptor activator of nuclear factor-kappa B ligand (RANKL), has been related to the occurrence of osteonecrosis of the jaws. Thus, the aim of this study was to review the literature from clinical case reports, regarding the type of patient and the therapeutic approach used for osteonecrosis of the jaws induced by chronic use of Denosumab. Material and Methods For this, a literature review was performed on PubMed, Medline and Cochrane databases, using the keywords “Denosumab” “anti-RANK ligand” and “Osteonecrosis of jaw”. To be included, articles should be a report or a serie of clinical cases, describing pat…
The role of bisphosphonates in the treatment of painful metastatic bone disease: a review of phase III trials.
1998
Metastatic bone disease is a frequent cause of morbidity in advanced cancer patients with a subsequent high incidence of skeletal complications (fractures, hypercalcemia, spinal cord compression) and severe pain. The osteolytic process is mainly characterized by an osteoclastic activity of bone resorption and inflammatory activity provoked by various cytokines and prostaglandins. Bisphosphonates represent a new class of drugs with inhibitory activity on bone resorption and on inflammatory processes which revealed themselves to be efficacious in a series of clinical conditions such as tumour-induced hypercalcemia, Paget's disease, osteoporosis and metastatic bone disease. The aim of this rev…
Treatment of skeletal impairment in patients with endogenous hypercortisolism: when and how?
2014
Guidelines for the management of osteoporosis induced by endogenous hypercortisolism are not available. Both the American College of Rheumatology and the International Osteoporosis Foundation recommend to modulate the treatment of exogenous glucocorticoid-induced osteoporosis (GIO) based on the individual fracture risk profile (calculated by FRAX) and dose of glucocorticoid used, but it is difficult to translate corticosteroid dosages to different degrees of endogenous hypercortisolism, and there are no data on validation of FRAX stratification method in patients with endogenous hypercortisolism. Consequently, it is unclear whether such recommendations may be adapted to patients with endoge…
New formulation with potential for the prevention and treatment of osteoporosis and osteoarthritis
2013
ObjectiveThe aim of this study was to investigate the effect of a new formulation in a combined rat model of Osteoporosis (OP) and Osteoarthritis (OA). The nutraceutical formulation (BIS073) contai...
Biomechanical Study of the Osteoporotic Spine Fracture: Optical Approach
2021
Background and objectives: Osteoporotic spine fractures represent a significant factor for decreasing quality of life in the elderly female population. Understanding the mechanisms involved in producing these fractures can improve their prevention and treatment. This study presents a biomechanical method to produce a vertebral fracture, conducted on a human spine segment, observing the displacements and strains in the intervertebral disc, endplate, and vertebral body. Materials and Methods: We performed two tests, one corresponding to an extension loading, and the second to an axial loading. Results: The maximum displacement in the target vertebral body presented higher values in the case o…
