Prospective comparison of CT angiography of the legs with intraarterial digital subtraction angiography.

The aim of this study was to determine the accuracy of CT angiography (CTA) with a single spiral acquisition for the diagnosis of arterial stenoses and occlusion in patients with peripheral vascular occlusive disease.In a prospective study, intraarterial digital subtraction angiography and i.v. CTA from the groin to the lower calves were performed on 50 patients with vascular occlusive disease. Maximum-intensity-projection images in multiple views were produced. Renal transplantation followed by immunosuppression with cyclosporine (whole-blood levels 200-300 micrograms/l) and methylprednisolone (4 mg daily) was performed in a 54-year-old man with chronic glomerulonephritis. Three years later rapidly progressive arterial obstructive disease (peripheral type) developed. Parenteral treatment with prostaglandins, calcium antagonists and nitrates, as well as a lumbar sympathectomy, was unsuccessful so that, in rapid succession, several amputations on upper and lower limbs became necessary. Purpose To evaluate different rendering techniques of CT data for the assessment of long vessel segments in peripheral vascular occlusive disease. Material and methods 40 CT angiograms (aortoiliac: n = 20, leg arteries: n = 20) were viewed using three different rendering techniques: 1, maximum intensity projection (MIP); 2, volume rendering (VR); 3, shaded surface display (SSD). CT angiograms were obtained in 6 or 8 projections. Axial cross-section images were analysed using an interactive cine mode. Intraarterial DSA was the standard in all cases. Angiographic studies of the arteries adjoining the knee in 25 patients show extensive kinking and stenoses of the popliteal artery and less frequently of the distal femoral artery during flexion of the knee joint. This is due to the loss of elasticity with increasing age forcing the vessel into a tortuous course during shortening of the pathway of the popliteal artery with knee flexion. Independent of the principle of the different stents available they probably will not increase the contractility of the stented vessel in the longitudinal axis. Bakalaura darba tēma ir “Līdzestība ārstēšanā pacientiem ar perifēro artēriju okluzīvām saslimšanām”. Darbs sastāv no 41 lappusēm, 6 attēliem, 1 tabulas un 6 pielikumiem. Pētījuma mērķis ir noskaidrot pacientu līdzestību perifēro artēriju okluzīvo saslimšanu ārstēšanā. Pētījumā izmantota kvantitatīvā pētniecības metode – aptaujas anketa. Pētījumam izvirzīta hipotēze – pacientu ar perifēro artēriju okluzīvo saslimšanu līdzestība ārstēšanas procesā ir daļēji apmierinoša. Pacienti ir līdzestīgi medikamentozai terapijai, bet nevēlas mainīt dzīvesveida un uztura paradumus.
