Interleukin 12 induces activation of fibrinolysis and coagulation in humans

Interleukin 12 (IL-12) has potential efficacy in malignant, infectious and allergic diseases. Its side-effects include activation of coagulation and fibrinolysis, as documented in chimpanzees. We assessed the coagulative and fibrinolytic response in 18 patients with renal cell carcinoma after subcutaneous injection of 0.5 microg/kg recombinant human IL-12. IL-12 induced a fibrinolytic response in 17 patients (94%): plasmin-alpha2-anti-plasmin complexes (PAPc) increased from 11.8 +/- 6.6 nmol/l (mean +/- SD) to a maximum of 18.8 +/- 7.4 nmol/l at 72 h. Possibilities and necessities of parenteral nutrition with amino acid mixtures.

In recent years, there have been conclusive findings in the field of maintaining homeostasis following trauma, bleeding, nutritional and metabolic disturbances and infections. These findings can help save the lives of an increasing number of injured and sick by enlarging the therapeutic possibilities which have previously been unimaginable. This development has been made possible by the good cooperation of both basic research and clinical staff whose common aim was to achieve advances in this field of intensive care. The aim of the present study is to make a relevant theoretical and practical contribution to the widening possibilities and necessities in this field.

Role of the gamma-glutamyl cycle in the regulation of amino acid translocation

Amino acid translocation was studied in the mammary gland of lactating rats and in the placenta of pregnant rats. The uptake of amino acids by the mammary gland is maximal on days 10-14 of lactation and is minimal on days 19-21. However, on day 19 maximal uptake can be restored by injection of 1) small amounts of gamma-glutamyl amino acids, 2) 5-oxoproline, and 3) an inhibitor of 5-oxoprolinase. A severe decrease in uptake of amino acids at the peak of lactation is provoked by anthglutin, an inhibitor of gamma-glutamyltranspeptidase (GGT). During pregnancy various humoral and functional mechanisms were recognized, that regulate the water and electrolyte household. In preeclampsia, increased water-sodium retention results from a variety of pathophysiological changes in the endocrinological system. There are also organ-specific changes, such as renal clearance reduction up to 30% in comparison to normal pregnancy. For many years, physiologists expected to find additional cardial mechanisms that influence homeostasis. In 1981, de Bold [3] identified ANP a new humoral factor that leads to increased water and sodium excretion. The influence of ANP in preeclampsia has been the subject of controversial discussion.

Background Transarterial embolization (TAE) is an alternative procedure to repeat endoscopy or surgical intervention in the case of re-bleeding after primary endoscopic treatment. The aim of the study was to assess the Rockall score as a criterion for TAE in the case of re-bleeding after endoscopic treatment of non-variceal upper gastrointestinal bleeding (NVUGIB). Methods Out of the 673 patients who underwent emergent endoscopic hemostasis due to NVUGIB, 111 had a high risk of re-bleeding having a Forrest I-IIb ulcer and the Rockall score ≥ 5. Agouti-related peptide-expressing neurons are mandatory for feeding.

Multiple hormones controlling energy homeostasis regulate the expression of neuropeptide Y (NPY) and agouti-related peptide (AgRP) in the arcuate nucleus of the hypothalamus. Nevertheless, inactivation of the genes encoding NPY and/or AgRP has no impact on food intake in mice. Here we demonstrate that induced selective ablation of AgRP-expressing neurons in adult mice results in acute reduction of feeding, demonstrating direct evidence for a critical role of these neurons in the regulation of energy homeostasis.
