Il trattamento di ONJ è, dalla sua comparsa in letteratura, argomen- to molto dibattuto, prima, come osteonecrosi delle ossa mascellari (BRONJ) associata a bisfosfonati (BP), recentemente un’ulteriore complessità si è aggiunta con l’identificazione di numerose molecole di nuova generazione, diverse dai BP, in grado di favorire lo sviluppo di ONJ (definite nel presente testo “non-BRONJ”); queste molecole, nei pazienti oncologici per lesioni scheletriche, sono prescritte più o meno in associazione o in sequenza con i BP stessi (e.g. denosu- mab, inibitori delle tirosin-kinasi, anticorpi monoclonali anti-VEGF, inibitori di mTOR)1–3.

BRONJ is a serious oral complication of BP treatment involving the exposure of necrotic maxillary or mandibular bone. We investigated the pathogenetic hypothesis and new preventive measures for BRONJ in patients with bone metastases who received BP zoledronic acid (ZOL) and chemotherapy combined with the antiangiogenic agent bevacizumab (BEV). The pathogenesis of BRONJ is uncertain: the type of BP, duration of the drug use and the occurrence of tooth extractions or other invasive dental procedures during treatment seem to play a major role. Osteonecrosis of the jaws (ONJ) is an adverse side event of bisphosphonates and denosumab, antiresorptive agents that effectively reduce the incidence of skeletal-related events in patients with metastatic bone cancer and multiple myeloma. Recently there have been reports of osteonecrosis of the jaw (ONJ) in patients with chronic therapy with bisphosphonates (Bps). So far three drugs have been linked: Pamidronate of disodium, Zoledronic acid and Alendronate of sodium.
