Bisphosphonates are bone-turnover modulating drugs which are used in the management of a number of bone diseases ranging from osteoporosis to neoplasic pathology-associated osteolysis. In the last years a number of cases of osteonecrosis of the jaws associated with these drugs have been reported. In this review we analyze the cases published in the literature indexed from 2003 to December 2005. During this period 246 cases were reported, being more frequently associated with women in the sixth decade of life. The authors studied the micro - structural changes of aging long bones. The results show: an increase in the number of the osteons; a sensible decrease in their sectional area; a disappearance of the outer fundamental system until the sixth decade, which undergoes no significant changes afterwards. Whereas the area of the haversian canals and the percentage of the haversian system undergo no significant changes. The data suggest that the skeletal remodeling tends to keep constant, during the lfetime, the quota of the haversian system and the relation between the latter and the interstital system, through an increase in numebr of the osteons which involves a decrease in their size. Bisphosphonates offer a significant improvement in the quality of life for cancer patients; these potent inhibitors of bone resorption have been shown to markedly reduce the morbidity frequently resulting from bone metastases. Despite the success of bisphosphonates as therapeutic agents, however, toxicity in the form of osteonecrosis of the jaw (ONJ) is a rare complication whose incidence rate has climbed in recent years.
