Diaphragmatic surgery, a fundamental step of advanced stage ovarian cancer (ASOC) debulking surgery, is associated with a high post-operative complication incidence, which is supposedly reduced with thoracostomy tube placement. A low-pressure reservoir for urine is created by antimesenteric splitting and side-to-side anastomosis of the rectosigmoid, the expectation being to obtain better continence rates and better protection of the upper tract than are achievable by ureterosigmoidostomy. Between 1990 and August 1993 the procedure was performed in 73 patients (59 adults and 14 children) whose mean age was 43.5 years. The indications were malignancy (n = 55), bladder exstrophy/epispadias (n = 14), trauma (n = 3), and sinus urogenitalis (n = 1). Transrenal ureteral occlusion using a detachable balloon

Percutaneous transrenal ureteral occlusion using a detachable balloon filled with silicone was performed in 20 patients. The main indications were palliation of large urinary fistulas and as a treatment of last resort for severe dysuria in patients with advanced pelvic malignancies. In 6 patients the contralateral kidney was rendered nonfunctional to obviate the need for bilateral nephrostomy. Due to frequent obstruction of the tube, ureteral blockage in 3 became insufficient. Two dislocated balloons were extracted percutaneously and replaced. In order to achieve effective interruption of urinary flow down the ureter, well-functioning external nephrostomy drainage is necessary.

Anastomotic leakage is one of the major complications occurring after anterior resection of rectum. A defunctioning stoma is usually created routinely or on surgeons' discretion. The aim of this study was to investigate the usefulness of temporary ileostomy to prevent anastomotic leakage comparing the postoperative course of patients with and without defunctioning loop ileostomy.Patients that underwent anterior resection of rectum were recruited. 140 patients were enrolled and divided in two groups: patients without and with defunctioning loop ileostomy. Outcome and attitudes toward home tracheostomy ventilation of consecutive patients: a 10-year experience.

Summary Objectives To describe survival, predictors of long-term outcome and attitudes in patients treated at home by tracheostomy-intermittent positive-pressure ventilation (TIPPV) for respiratory failure during a 10-year period (1995–2004). Methods Seventy-seven consecutive patients were treated by TIPPV at home. Patients were divided into three groups: neuromuscular, pulmonary, and non-pulmonary patients.
