Is faster better? A randomised crossover study comparing algorithms for closed-loop automatic oxygen control

2019

ObjectiveClosed-loop automatic control (CLAC) of the fractional inspired oxygen (FiO2) improved oxygen administration to preterm infants on respiratory support. We investigated whether a revised CLAC algorithm (CLACfast, ≤2 FiO2 adjustments/min), compared with routine manual control (RMConly), increased the proportion of time with arterial haemoglobin oxygen saturation measured by pulse oximetry within prespecified target ranges (Target%) while not being inferior to the original algorithm (CLACslow: ≤0.3 FiO2 adjustments/min).DesignUnblinded randomised controlled crossover study comparing three modes of FiO2 control in random order for 8 hours each: RMC supported by CLACfast was compared wi…

Endurance training in patients with chronic obstructive pulmonary disease: a comparison of high versus moderate intensity.

2000

Abstract Gimenez M, Servera E, Vergara P, Bach JR, Polu J-M. Endurance training in patients with chronic obstructive pulmonary disease: a comparison of high versus moderate intensity. Arch Phys Med Rehabil 2000;81:102-9. Purpose: To create a maximum tolerated 45-minute aerobic training program for patients with chronic obstructive pulmonary disease (COPD) and to compare its outcomes with those of commonly prescribed moderate exercise. Design: Prospective, randomized trial. Setting: A work physiology laboratory. Patients and Methods: The maximum exercise intensities that 7 COPD patients could sustain for 45 minutes were determined on a bilevel exercise ergometer. The patients then exercised …

Hemodynamic responses to speech and music in newborn infants

2009

We used near‐infrared spectroscopy (NIRS) to study responses to speech and music on the auditory cortices of 13 healthy full‐term newborn infants during natural sleep. The purpose of the study was to investigate the lateralization of speech and music responses at this stage of development. NIRS data was recorded from eight positions on both hemispheres simultaneously with electroencephalography, electrooculography, electrocardiography, pulse oximetry, and inclinometry. In 11 subjects, statistically significant (P < 0.02) oxygenated (HbO(2)) and total hemoglobin (HbT) responses were recorded. Both stimulus types elicited significant HbO(2) and HbT responses on both hemispheres in five subjec…

Effects of mandibular setback surgery on upper airway dimensions and their influence on obstructive sleep apnoea - a systematic review.

2015

Abstract Background Mandibular setback used to be the traditional treatment of choice for correcting mandibular prognathism. Nowadays, bimaxillary surgery is preferred. Several authors have asserted that mandibular setback causes a relative narrowing of the upper airway (UA) that could trigger obstructive sleep apnoea (OSA); however, its potential role in OSA development is still much debated. Another controversial subject is whether changes in airway space caused by the procedure are permanent. Objectives To ascertain the consequences for UA size and shape of mandibular setback surgery in comparison with bimaxillary surgery (maxillary advancement with Le Fort I and mandibular setback), and…

Challenges in automated estimation of capillary refill time in dogs

2018

Capillary refill time (CRT) is a part of the cardiorespiratory examination in dogs. Changes in CRT can reflect pathological conditions like shock or anemia. Visual CRT estimation has low repeatability; therefore, optical systems for automated estimation have recently appeared. Since existing systems are unsuitable for use in dogs, we designed a simple, small and portable device, which could be easily used at veterinary clinic. The device was preliminarily tested on several measurement sites in two dogs. Not all measurement sites were suitable for CRT measurements due to underlying tissue optical and mechanical properties. The CRT measurements were possible on the labial mucosa, above the st…

Diammine{N-[2-(hydroxyimino)propionyl]-N′-[2-(oxidoimino)propionyl]propane-1,3-diaminido-κ4N,N′,N′′,N′′′}iron(III)

2012

In the title compound, [Fe(C9H13N4O4)(NH3)2], the Fe(III) atom, lying on a mirror plane, is coordinated by four N atoms of a triply deprotonated tetra-dentate N-[2-(hy-droxy-imino)-propion-yl]-N'-[2-(oxidoimino)-propion-yl]propane-1,3-diaminide ligand in the equatorial plane and two N atoms of two ammonia mol-ecules at the axial positions in a distorted octa-hedral geometry. A short intra-molecular O-H⋯O hydrogen bond between the cis-disposed oxime O atoms stabilizes the pseudo-macrocyclic configuration of the ligand. In the crystal, mol-ecules are linked by N-H⋯O hydrogen bonds into a three-dimensional network. The ligand has a mirror-plane symmetry. One of the methyl-ene groups of the pro…

Infection of the upper extremity by Mycobacterium marinum in a 3-year-old boy--diagnosis by 16S-rDNA analysis.

1996

A 3-year-old boy developed several subcutaneous nodular lesions on his right arm. Based on the histological examination of one of these nodules furunculosis was suspected and cefuroxime was tentatively given. However, acid-fast bacilli were then detected in the tissue specimen and a few colonies of acid fast, gram-positive rods grew on blood agar. Definitive species diagnosis (Mycobacterium marinum) was rapidly achieved by automated sequencing of amplified 16S-rDNA and antimicrobial therapy was adjusted according to the available literature. After 3 weeks of treatment with clarithromycin, rifampicin and protionamid regression of the nodular lesions was evident.

Amidrazone Complexes from a Cascade Platinum(II)-Mediated Reaction between Amidoximes and Dialkylcyanamides

2013

The aryl amidoximes R'C6H4C(NH2)═NOH (R' = Me, 2a; H, 2b; CN, 2c; NO2, 2d) react with the dialkylcyanamide platinum(II) complexes trans-[PtCl2(NCNAlk2)2] (Alk2 = Me2, 1a; C5H10, 1b) in a 1:1 molar ratio in CHCl3 to form chelated mono-addition products [3a-h]Cl, viz. [PtCl(NCNAlk2){NH═C(NR2)ON═C(C6H4R')NH2}]Cl (Alk2 = Me2; R' = Me, a; H, b; CN, c; NO2, d; Alk2 = C5H10; R' = Me, e; H, f; CN, g; NO2, h). In the solution, these species spontaneously transform to the amidrazone complexes [PtCl2{NH═C(NR2)NC(C6H4R')NNH2}] (7a-h; 36-47%); this conversion proceeds more selectively (49-60% after column chromatography) in the presence of the base (PhCH2)3N. The observed reactivity pattern is specific …

Synthesis, structure and magnetic properties of oligometallic systems derived from di- and trinuclear copper(ii) amido-oximate complexes

2008

Three heterometallic complexes [M(H(2)O)(n)][Cu(3)L(2)(H(2)O)] (M = Mn(2+), Co(2+) or Ba(2+)) and one dinuclear compound (CuDien)(CuL{H(2)O}) were prepared by interaction of anionic compounds Cu(3)L(2)(2-) or CuL(2-) with the corresponding cations (H(4)L = 1,9-dicyano-1,9-bis(hydroximino)-3,7-diazanonane-2,8-dione; Dien = 1,5-diamino-3-azapentane). The complexes [M(H(2)O)(n)][Cu(3)L(2)(H(2)O)] have a polymeric structure, formed via oligomerization of Cu(3)L(2)(2-) units and additionally, in the case of the Ba-salt, by binding of Cu(3)L(2)(2-) units through Ba(2+). Antiferromagnetic interactions occur in all the complexes, while for [Co(H(2)O)(6)][Cu(3)L(2)(H(2)O)] there is evidence of some …
