Left ventricular ejection fraction recovery in patients with heart failure treated with intravenous iron: a pilot study
2016
Aims In patients with heart failure with reduced ejection fraction (HFrEF) and iron deficiency, treatment with intravenous iron has shown a clinical improvement regardless of anaemic status. Cardiac magnetic resonance (CMR) T2* sequence has shown a potential utility for evaluating myocardial iron deficiency. We aimed to evaluate whether T2* sequence significantly changes after ferric carboximaltose (FCM) administration, and if such changes correlate with changes in left ventricle ejection fraction (LVEF). Methods and results In this pilot study, we included eight patients with chronic symptomatic (New York Heart Association II–III) HFrEF and iron deficiency. A CMR, including T2* analysis, w…
0283: Correlation between N-terminal pro-natriuretic peptide levels and left atrial size in patients with left ventricular systolic dysfunction
2014
BackgroundElevated N-terminal Pro B-type Natriuretic Peptide (NT Pro-BNP) levels are correlated with the severity of left ventricular systolic dysfunction and left ventricular filling alterations. Besides, left atrial (LA) size is believed to reflect chronic diastolic dysfunction.AimEvaluate the relationship between NT Pro-BNP and LA size.Methods81 patients (77.7% males; mean age, 61.55±15 years), with left ventricular systolic dysfunction (left ventricular ejection fraction [LVEF] 20cm2 was 1188pg/mL (area under the receiver-operating characteristic curve [AUC] 0.83 [0.71–0.93], p<0.0001, sensitivity 77.6%, specificity 75%).ConclusionThe present study shows that plasma NT Pro-BNP levels is…
Treatment outcomes in hepatitis C virus genotype 1a infected patients with and without baseline NS5A resistance‐associated substitutions
2020
Background&aims The presence of baseline resistance-associated substitutions (RASs) reduced sustained virologic response (SVR) rates in chronic hepatitis C virus (HCV) genotype 1a infected patients treated with Elbasvir/Grazoprevir (EBR/GZR). This study aimed to evaluate the frequency of NS5A RASs and treatment outcomes in patients for whom EBR/GZR was intended. Methods We sequenced NS5A in 832 samples from German genotype1a-infected DAA-naive patients population-based, which were collected in the European Resistance Database. Treatment outcomes and clinical parameters were evaluated in 519 of these patients retrospectively. Results Overall, 6.5% of patients harbored EBR-specific NS5A RASs …
Extraperitoneal ileal conduit
2011
Today incontinent urinary diversion continues to be the most widely used diversion technique. A current Medicare evaluation showed that 80% of patients undergoing urinary diversion in the USA are subject to reconstruction by means of an ileal conduit [1]. In particular, older patients, patients with comorbidities, and women more frequently receive an incontinent urinary diversion [2,3]. Even with improved perioperative clinicalcare pathways, urinary diversion remains a morbid procedure. Complications are still common after urinary diversion with ≈ 27% patients re-admitted in the first 90 days after BJUI
WHO needs high FIO2?
2017
World Health Organization and the United States Center for Disease Control have recently recommended the use of 0.8 FIO2 in all adult surgical patients undergoing general anaesthesia, to prevent surgical site infections. This recommendation has arisen several discussions: As a matter of fact, there are numerous studies with different results about the effect of FIO2 on surgical site infection. Moreover, the clinical effects of FIO2 are not limited to infection control. We asked some prominent authors about their comments regarding the recent recommendations.
Impact of direct-acting antivirals (DAAs) on cardiovascular diseases in patients with chronic hepatitis C
2021
INTRODUCTION: In the last years the Hepatitis C virus (HCV) infection was a relevant public health problem due to the large number of affected people worldwide and the impact on hepatic and extrahepatic complications. The availability of direct-acting antivirals (DAAs) and the very high rate of sustained virological response (SVR) after treatment has radically changed the course of HCV chronic infection. EVIDENCE ACQUISITION: We searched PubMed for articles published between January 1, 1995, through December 31, 2020, in English language. EVIDENCE SYNTHESIS: Robust evidence showed a close link between HCV infection and development of cardiovascular disease (CVD), as result of the atherogeni…

Mesencephalic ependymal cysts: treatment under pure endoscopic or endoscope–assisted keyhole conditions
2008
Object Primary intracranial ependymal cysts are extremely rare. Similar to congenital intraparenchymal cysts in the mesencephalon they usually occur with symptoms of an occlusive hydrocephalus or symptoms like Parinaud syndrome, dizziness, or gait disturbance. The objective of this study was to evaluate the surgical methods for the treatment of these cysts and the clinical outcome of the patients. Methods The authors present the clinical records of 8 patients who were treated in their department for symptomatic mesencephalic ependymal cysts in the past 10 years. The patient age ranged from 22 to 60 years with a mean age of 44 years. In 4 cases the authors performed a suboccipital infratent…
Evaluation of interface pressure and temperature management in five wheelchair seat cushions and their effects on user satisfaction
2020
Abstract Background Factors such as the manufacturing materials, shape or even the mechanical and thermal response of sitting Pressure Redistribution Support Surfaces (PRSS) can be potential contributors to pressure ulcers. However, few studies have compared a number of characteristics of the most frequently used devices. Objective To compare three potential contributors to pressure ulcers in five commercial PRSS: pressure redistribution, temperature and perceived comfort. Method: Study with a cross-over randomized design in healthy volunteer participants. Data was collected in a temperature and relative humidity controlled environment. To assess thermal response, the temperature (Flir-E60)…
