Prescription of automatic bilevel ventilation (AutoBI) in sleep-disordered breathing: analysis according to diagnosis and occurrence of comorbidities

CPAP is the first-choice treatment for obstructive sleep-disordered breathing (O-SDB), while the criteria to prescribe AutoBI are undefined. Trials of AutoBI ventilation in patients (pts) treated for O-SDB from January 2015 to October 2016 (64 out of 242 titrations) were reviewed to assess: a) the clinical characteristics of these pts, b) the reason to shift from CPAP to AutoBI, c) the compliance to prescribed treatment. AutoBI was used in cases of intolerance to high therapeutic CPAP levels (n=21) or incomplete resolution of SDB on CPAP (n=43). IntroductionFor Major Depression, current guidelines recommend treatment durations of 3-8 weeks until optimisation in case of insufficient outcome. Many retrospective studies indicate that improvement (HAMD-17 decrease ≥20%) occurs usually within 10-14 days and that non-improvement after 14 days of treatment is highly predictive for poor clinical outcome.MethodsIn level 1 of the EMC trial, non-improvers after 14 days of antidepressant treatment will be randomised to “early medication change” (EMC) strategy or treatment according to current guidelines (TAU). Intermittent hyperammonemic encephalopathy after ureterosigmoidostomy: spontaneous onset in the absence of hepatic failure

Intermittent hyperammonemic encephalopathy after ureterosigmoidostomy is a rare, but if unrecognized, potentially lethal condition. Ureterosigmoidostomy was performed in a male patient with bladder extrophy. After 35 years, he developed hyperammonemic encephalopathy. Diagnostic procedures did not reveal hepatic nor metabolic disorders. Despite administration of preventive medical treatment, several episodes recurred. Low-dose aspirin (ASA, 75 - 325 mg/day) is widely used for the primary and secondary prevention of cardiovascular (CV) diseases. However, the value of primary prevention ASA is uncertain as the reduction in occlusive events needs to be weighed against the significant increase in major bleedings. Prevention with antisecretory drugs has been proposed to reduce the incidence of ASA-induced gastrointestinal (GI) bleedings, but non-adherence to gastro-protection is of concern, as it significantly increases the risk of upper GI adverse events. Coronary Spasm in Patients Treated by Percutaneous Transluminal Coronary Angioplasty

The appearance of coronary spasm during PTCA was analyzed in 140 consecutive patients with stable and unstable angina. Coronary spasm was found in 27 patients (19%) and was more common in unstable than in stable angina pectoris (22 versus 5 patients). While coronary spasm could be seen in the first coronary angiogram in 5/27 patients, it developed during the diagnostic procedure in 6/27 patients. In 16/27 patients coronary spasm was induced by the balloon or the guide wire itself.

Severe tricuspid regurgitation (TR) has long been neglected despite its well-known association with mortality. While surgical mortality rates remain high in isolated tricuspid valve surgery, interventional TR repair is rapidly evolving as an alternative to cardiac surgery in selected patients at high surgical risk. The Assessment of General Health Status in Patients with Odonto-Periodontal Pathology and Diabetes Mellitus

Abstract The present research attempts to assess the general health status of the studied group of patients by applying the questionnaire method. The research group has been composed of 210 subjects, from both urban and rural areas, having periodontal pathologies and disorders of the glucidic metabolism. In order to obtain information about the general health condition as well as the orodental hygiene, we have applied the questionnaire method, this being well individualized and adapted to be relevant to the present study. It was highlighted that patients with odonto-periodontal pathology and diabetes gave a more affected general health compared to those without diabetes.
