Pharmacological Therapy in Children with Atrial Fibrillation and Atrial Flutter

2008

Heart rhythm disorders in children are not different, on electrocardiographic trace, from heart rhythm disorders in adults with the exception of incidence which is different according to the age. Paticularly, atrial flutter (FlA) and fibrillation (FA) are very uncommon arrhythmias in the general pediatric population. Generally atrial fibrillation and atrial flutter, in our experience, is a temporary heart rhythm disturbance connected to specifical and resovable reasons with the exception of Fontains surgical correction of congenital heart diseases or cardiopathies with dilatation of both atria. Presenting symptoms, symptom history (e.g., frequency, duration, and severity), risk assessment, …

Jugular pacing lead extraction with laser sheath: a case report.

2010

Over the past 20 years, the number of patients with pacemakers (PM) or implantable cardioverter defibrillators has risen markedly; consequently, an increasing number of lead-removal procedures have become necessary. A 64-year-old woman presenting with an infected device pocket and positive bacterial cultures ( Staphylococcus aureus ) was admitted to our department for lead removal; in 1991, she underwent VVI PM implantation for atrioventricular II degree Mobitz 1 block, and a unipolar lead was introduced via the left jugular vein. The procedure was performed in our Electrophysiology Lab with a cardiac surgeon on standby, using an excimer laser system emitting the energy at the tip of a flex…

Role of cardiac dyssynchrony and resynchronization therapy in functional mitral regurgitation

2016

Functional mitral regurgitation (FMR) is a common complication of left ventricle (LV) dysfunction and remodelling. Recently, it has been recognized as an independent prognostic factor in both ischaemic and non-ischaemic LV dysfunctions. In this review article, we discuss the mechanisms through which cardiac dyssynchrony is involved in FMR pathophysiologic cascade and how cardiac resynchronization therapy (CRT) can have therapeutic effects on FMR by reverting specific dyssynchrony pathways. We analyse recent clinical trials focusing on CRT impact on FMR in 'real-world' patients, the limits and future perspectives that could eventually generate new predictors of CRT response in terms of FMR r…

Focus on right ventricular outflow tract septal pacing

2013

SummaryExperimental and clinical studies have shown that right ventricular apical pacing may result in long-term deleterious effects on account of its negative impact on left ventricular remodeling through desynchronization. This risk appears more pronounced in patients with even moderate left ventricular dysfunction and generally occurs after at least 1 year of pacing. As right ventricular apical pacing may be associated with the development of organic mitral insufficiency, other sites that allow for more physiological stimulation, such as right ventricular outflow tract septal pacing, have been developed, with good feasibility and reproducibility. However, the prospective randomized studi…

Modifications in Ventricular Fibrillation and Capture Capacity Induced by a Linear Radiofrequency Lesion

2012

[EN] Introduction and objectives: An analysis was made of the effects of a radiofrequency-induced linear lesion during ventricular fibrillation and the capacity to capture myocardium through high-frequency pacing. Methods: Using multiple epicardial electrodes, ventricular fibrillation was recorded in 22 isolated perfused rabbit hearts, analyzing the activation maps upon applying trains of stimuli at 3 different frequencies close to that of the arrhythmia: a) at baseline; b) after radio-frequency ablation to induce a lesion of the left ventricular free wall (length=10 [1] mm), and c) after lengthening the lesion (length=23 [2] mm). Results: Following lesion induction, the regularity of the r…

Muscle cramping in the marathon: dehydration and electrolyte depletion vs. muscle damage

2020

Martinez-Navarro, I, Montoya-Vieco, A, Collado, E, Hernando, B, Panizo, N, and Hernando, C. Muscle Cramping in the marathon: Dehydration and electrolyte depletion vs. muscle damage. J Strength Cond Res XX(X): 000-000, 2020-Our aim was to compare dehydration variables, serum electrolytes, and muscle damage serum markers between runners who suffered exercise-associated muscle cramps (EAMC) and runners who did not suffer EAMC in a road marathon. We were also interested in analyzing race pacing and training background. Nighty-eight marathoners took part in the study. Subjects were subjected to a cardiopulmonary exercise test. Before and after the race, blood and urine samples were collected and…

Stymulacja pęczka Hisa u osoby starszej z proksymalnym blokiem przedsionkowo-komorowym po zawale mięśnia sercowego

2018

Stymulacja pęczka Hisa (HBP) umożliwia synchronizację przedsionkowo-komorową i międzykomorową. W artykule przedstawiono przypadek 92-letniego mężczyzny w bardzo dobrym stanie psychofizycznym z napadowym blokiem przedsionkowo-komorowym II stopnia i utrwalonym długim odstępem PR w przebiegu zawału mięśnia sercowego bez uniesienia odcinka ST-T, leczonego angioplastyką gałęzi okala…

Prekäre Räume

2020

Recent events in the Mediterranean call up for a radical questioning of our categories and representations. Sinking boats and emergency landings, reception policies and detention centres, crossings, deviations and refoulements symbolize an era when mobility is fatally related to security policies and new biopolitical regimes. Migrant bodies crossing the Mediterranean address a demand and a challenge that cultural geography is urged to accomplish. The aim of this paper is therefore to rethink spatiality in an ethical sense. Drawing on the philosophy of Emmanuel Lévinas and Judith Butler, we will try to imagine a new spatiality starting from bodies, displacements, and the complex open weave p…
