2008

In recent years, there has been a considerable increase in the number of procedures carried out under regional anesthesia. The techniques used can be associated with a number of complications, which should be understood so that they can be recognized and managed appropriately. The overall incidence of reported complications associated with these techniques is low and therefore, with currently available data, we can only have an approximate idea of their incidence. The objective of this study is to systematically describe the complications that may arise from the use of neuraxial and peripheral regional anesthesia techniques.

Regional anesthesia and chronification of acute postoperative pain

2014

Chronification of acute postoperative pain is the consequence of a number of factors; the article mentions different preemptive strategies that may put a limit to its development. Certain anesthetic and analgesic techniques have been assessed over the last years, and most articles highlight the usefulness of carrying out regional anesthesia techniques from the intraoperative period through several days into the postoperative period. Preventing the patients from suffering acute postoperative pain is one of the most appropriate tools for stopping the activation of the mechanisms involved in pain chronification. None of the regional techniques are equally valid and applicable to all surgical p…

2021

The authors regret that errors were present in the above article. On page 1174, in the second paragraph of the Statistical methods section, the second sentence should read as follows: The incidence of difficult intubation was determined including those whose tracheas were already intubated and is reported as a percentage with a 95% exact binomial CI. On page 1175, in the third paragraph of the Statistical methods section ‘mean standardised difference (MSD)’ should read ‘standardised mean difference (SMD)’ The authors would like to apologise for any inconvenience caused.

Are we overusing abdominal computed tomography scans in young patients referred in an emergency for acute abdominal pain?

2022

Purpose: The primary objective was to assess the frequency of appropriateness of computed tomography (CT) for acute abdominal pain (AAP) in the emergency department; the secondary aim was to compare the diagnostic accuracy of ultrasound (US) and CT in the diagnosis of the aetiology of AAP for diseases that can be diagnosed by US; and the third objective was to assess extent to which inappropriate CT examinations for AAP result in ionizing radiation exposure. Material and methods: In this retrospective single-centre study, we included patients aged between 15 and 46 years referred to the emergency department for AAP in 2016 and submitted to abdominal CT scans, collecting a total of 586 patie…

Pain modulates early sensory brain responses to task‐irrelevant emotional faces

2023

Background Pain can have a significant impact on an individual's life, as it has both cognitive and affective consequences. However, our understanding of how pain affects social cognition is limited. Previous studies have shown that pain, as an alarm stimulus, can disrupt cognitive processing when focal attention is required, but whether pain also affects task-irrelevant perceptual processing is unclear. Methods We examined the effect of laboratory-induced pain on event-related potentials (ERPs) to neutral, sad, and happy faces before, during, and after a cold pressor pain. ERPs reflecting different stages of visual processing (P1, N170, and P2) were analyzed. Results Pain decreased the P1 …

Brain excitability in migraine: Hyperexcitability or inhibited inhibition?

2007

While migraine has been considered for a long time a disease of the head vascular structures, it has been more and more evident that the involvement of the cerebral cortex has a primary role in the cascade of events which trigger the migraine attacks. Neurophysiological studies gave an important contribution to our knowledge of the mechanisms underlying the migraine disease

Autologous conditioned serum for chronic pain in patients with osteoarthritis: A feasibility observational study

2022

Background Autologous conditioned serum is a product of blood origin, with fragmented evidence of therapeutic properties in osteoarthritis chronic pain. This pilot observational prospective study aimed to evaluate the feasibility of a treatment with conditional autologous serum (ACS) in patients with severe chronic pain and grade I-III osteoarthritis and to describe its cytokine content. Methods We prospectively collected data on consecutive patients affected by osteoarthritis grade I to III and treated with four weekly injections of ACS at our outpatient pain service. The primary outcome was pain intensity, measured with the visual analogic scale (VAS). Additional outcomes were symptoms ev…
