In recent years, there have been conclusive findings in the field of maintaining homeostasis following trauma, bleeding, nutritional and metabolic disturbances and infections. These findings can help save the lives of an increasing number of injured and sick by enlarging the therapeutic possibilities which have previously been unimaginable. This development has been made possible by the good cooperation of both basic research and clinical staff whose common aim was to achieve advances in this field of intensive care. The aim of the present study is to make a relevant theoretical and practical contribution to the widening possibilities and necessities in this field.

Nutrition in Patients with Head and Neck Cancer. Malnutrition is common in patients with head and neck cancer. Due to natural course of disease or therapy, malnutrition may increase. This leads to a reduced prognosis, quality of life, and should be considered in early stages of the disease. First choice is an enteral feeding via a nasogastral feeding tube or a PEG. In special cases a parenteral nutrition may be necessary.

Malnutrition is a common consequence of inflammatory bowel disease (IBD). Diet has an important role in the management of IBD, as it prevents and corrects malnutrition. It is well known that diet may be implicated in the aethiology of IBD and that it plays a central role in the pathogenesis of gastrointestinal tract disease. Delayed-Onset Superior Mesenteric Artery Syndrome Presenting as Oesophageal Peptic Stricture

Superior mesenteric artery (SMA) syndrome is an infrequent cause of vomiting and weight loss due to compression of the third part of the duodenum by the SMA. We describe the case of a 17-year-old woman, admitted to our department for progressive dysphagia and severe weight loss due to an oesophageal peptic stricture, caused by chronic acid reflux secondary to duodenal compression by the SMA. Symptoms improved after (par)enteral nutrition and repeated oesophageal dilatation, thus supporting the role of intensive medical and endoscopic intervention as an alternative to surgery, at least in some cases.

Stability of a Parenteral Formulation of Betamethasone and Levobupivacaine

Background: The therapeutic management of syndromes presenting simultaneously pain and inflammation often requires the administration of anesthetic and corticosteroid drugs by epidural administration. In this article, we studied a mixture that combines betamethasone and levobupivacaine, which demonstrates prolonged analgesic effects. To our knowledge, the stability of such a mixture in epidural solution has not been examined. BACKGROUND In the intensive care unit (ICU), a fasting period is usually respected to avoid gastric aspiration during airway management procedures. Since there are no recognised guidelines, intensive care physicians balance the aspiration risk with the negative consequences of underfeeding.
