Atypical radiological presentation of multiple cystic brain metastases from lung cancer simulating neurocysticercosis.

Brain metastases (BMs) are usually characterised by vasogenic oedema and mass effect, but cystic appearance can rarely occur, mimicking parasitosis, such as neurocysticercosis (NCC). A woman in her mid-50s was admitted for dizziness and upper left extremity paresis. Neuroimaging showed multiple cystic lesions consistent with multiple stages of NCC evolution, and empiric albendazole was started, without any clinical improvement. A whole-body CT revealed a pulmonary lesion in the right superior lobe. Pathological analysis from brain specimen demonstrated a clear cell lung carcinoma. Endoscope-assisted microsurgery for cerebral aneurysms.

A total of 66 patients with intracranial aneurysms were endoscopically assisted treated during a 3 years period. Among those were five individuals with giant aneurysms and 27 patients with aneurysms of the posterior circulation. The endoscope was used only for checking the anatomical structures surround the aneurysms in 16 cases. In 43 patients the aneurysm sac was also dissected under endoscopical con-trol. Even the clipping procedure was performed in seven cases exclusively under endoscopical obser-vation. Fresh cell therapy followed by fatal coma

A 60-year-old woman received a 3-day course of nine injections of “fresh” cells from fetal lamb ovary, placenta, brain (hypothalamus) and liver. There were no immediate complications, but a few days later she developed headache, fever and hemiparesis. She subsequently fell into a coma and died 3 weeks after her fresh cell therapy and 2 weeks after the onset of her clinical symptoms. Autopsy revealed perivenous leucoencephalopathy with a probably steroid-treatment-induced paucity of perivascular inflammation. Distribution and course of cortico-respiratory projections for voluntary activation in man

The function and course of the cortico-respiratory projections in man are not yet well established. In 30 normal volunteers respiratory muscles were activated by magnetic stimulation of the motor cortex and the cervical and thoracic spinal roots with bilateral recordings from the respiratory muscles. Following cortical stimulation contralateral responses were obtained in all subjects during voluntary inspiration showing a mean latency and amplitude of 13.4±1.4ms/1.6±1.2 mV (stimulation of the left hemisphere) and 13.2±1.3ms/2.5±2.5 mV (stimulation of the right hemisphere). Slowed abduction saccades in bilateral internuclear ophthalmoplegia

Horizontal eye movements were investigated in 65 patients with bilateral internuclear ophthalmoplegia utilizing direct current electro-oculography. Abduction saccades were slowed in 35 patients (53.8%), being hypermetric in 65.7% of them. Slowing of abduction saccades is attributed to impaired inhibition of the tonic resting activity of the antagonistic medial rectus muscle. Experimental data indicate that this slowing results from a lesion of an uncrossed connection between the pontine reticular formation and the oculomotor nucleus. Abduction saccades in unilateral internuclear ophthalmoplegia

Horizontal eye movements were investigated in 60 consecutive patients with unilateral internuclear ophthalmoplegia utilizing direct current electrooculography. In nine patients additional conjugated slowing of ipsiversive saccades indicated the diagnosis of a one-and-a-half syndrome. Slowing of abduction saccades was bilateral in two patients and unilateral in 20 (ipsilateral to the MLF lesion in 17 patients and contralateral in three).
