Management and predictors of clinical events in 75 686 patients with acute myocardial infarction

2022

Background:Although mortality of patients with acute myocardial infarction (MI) has decreased substantially over the last few decades in many countries MI remains a major threat to public health. Aims:To assess the number and outcomes of patients hospitalized for acute MI in Poland in 2018 as well as proportions of patients participating in cardiac rehabilitation and undergoing invasive cardiac procedures following discharge. Methods:We used public databases. We included all patients hospitalized for acute MI in Poland in 2018 and assessed event-free survival along with uptake of invasive cardiac procedures and cardiac rehabilitation and consultations with cardiologists. Results: A total of…

Pełna trójnaczyniowa rewaskularyzacja wieńcowa z użyciem ultra niskich dawek kontrastu jako strategia zachowania rezydualnej funkcji nerek u chorego …

2018

Nefropatia indukowana kontrastem jest poważnym powikłaniem koronarografii i przezskórnych interwencji wieńcowych (PCI) u chorych z przewlekłymi schorzeniami nerek. Jedyną realną strategią zapobiegawczą przed jej rozwojem jest adekwatne nawodnienie wraz z ograniczeniem objętości stosowanego kontrastu. Ostatnio zaproponowano nowe techniki diagnostyczne i terapeutyczne polegające na stosowaniu ultra niskich dawek kontrastu, co więcej opisano zabiegi PCI bez użycia kontrastu. W niniejszym artykule prezentujemy przypadek dializowanego chorego z ostrym zespołem wieńcowym i ciężką niewydolnością jedynej nerki, ale z zachowaną rezydualną jej funkcją, u którego przeprowadzono diagnostykę i pełną tró…

Retrograde Chronic Total Occlusion Percutaneous Coronary Interventions: Predictors of Procedural Success From the ERCTO Registry

2022

Objectives: The aim of this study was to identify independent predictors of procedural success after retrograde chronic total occlusion (CTO) percutaneous coronary intervention (PCI). Background: Retrograde CTO PCI is an established technique, but predictors of success remain poorly understood. Methods: A multivariable logistic regression model was used to analyze potentially important demographic, clinical, anatomical, and technical aspects of retrograde CTO PCI cases uploaded to the multicenter European CTO (ERCTO) Club Registry. Results: In calendar years 2018 and 2019, 2,364 retrograde CTO PCI cases constituted the primary analysis cohort. A primary retrograde strategy was used in 1,953…

Post-traumatic Stress Disorder and Risk Factors in Patients With Acute Myocardial Infarction After Emergency Percutaneous Coronary Intervention: A Lo…

2021

This study aimed to investigate the status and risk factors of post-traumatic stress disorder (PTSD) in patients with acute myocardial infarction (AMI) after emergency percutaneous coronary intervention (PCI) in acute and convalescence phases. A longitudinal study design was used. Two questionnaire surveys were conducted in the acute stage of hospitalization, and 3 months after onset in patients. Logistic regression was used to analyze the risk factors for PTSD in AMI patients. The incidence of PTSD was 33.1 and 20.4% in acute and convalescent patients, respectively. The risk factors related to PTSD were door-to-balloon time (DTB) (≥92.6 min), left ventricular ejection fraction (LVEF) (&amp…

Gender-related differences in men and women with ST-segment elevation myocardial infarction and incomplete infarct-related artery flow restoration: a…

2018

Introduction: Little is known about gender-related differences in ST-segment elevation myocardial infarction (STEMI) and incomplete infarct-related artery (IRA) reperfusion after primary percutaneous coronary intervention (pPCI). Aim: To evaluate gender-related differences in clinical characteristics and prognosis in patients with STEMI and incomplete IRA reperfusion after pPCI. Material and methods: From 42,752 STEMI patients hospitalized between 2009 and 2011 in Poland we analyzed a group of 984 (36%) females and 1,746 (64%) males with less than Thrombolysis in Myocardial Infarction (TIMI) grade 3 flow following pPCI. Results: Women were older than men (72.0 ±11.3 vs. 64.0 ±11.7 years; p …

Correlation Between Smoking Paradox and Heart Rhythm Outcomes in Patients With Coronary Artery Disease Receiving Percutaneous Coronary Intervention

2022

BackgroundThe effect of smoking on short-term outcomes among patients with acute coronary syndrome (ACS) undergoing percutaneous coronary intervention (PCI) is controversial. However, little is known about the impact of smoking on long-term outcomes in patients with stable coronary artery disease (CAD) who receive PCI.MethodsA total of 2,044 patients with stable CAD undergoing PCI were evaluated. They were divided into two groups according to smoking status (current smokers vs. non-smokers). Baseline characteristics, exposed risk factors, angiographic findings, and interventional strategies were assessed to compare the long-term clinical outcomes between groups. Predictors for myocardial in…

Carga de comorbilidad y beneficio de la revascularización en ancianos con síndrome coronario agudo

2021

Introduction and objectives: To evaluate the interaction between comorbidity burden and the benefits of in-hospital revascularization in elderly patients with non-ST-segment elevation acute coronary syndrome (NSTEACS). Methods: This retrospective study included 7211 patients aged ≥ 70 years from 11 Spanish NSTEACS registries. Six comorbidities were evaluated: diabetes, peripheral artery disease, cerebrovascular disease, chronic pulmonary disease, renal failure, and anemia. A propensity score was estimated to enable an adjusted comparison of in-hospital revascularization and conservative management. The end point was 1-year all-cause mortality. Results: In total, 1090 patients (15%) died. Th…
