Abstract Background Gastroesophageal reflux (GER) and dysmotility are frequent in patients treated for esophageal atresia (EA). This aim of this study is to evaluate GER and dysmotility in young EA patients using pH–multichannel intraluminal impedance (pH-MII). Methods Fifteen patients with a mean age of 7.5 years (group 1) have been studied and compared with 15 children without congenital malformation, submitted to pH-MII for suspected GER (group 2). 1. The effects of pirenzepine and hyoscine on the peristaltic reflex were investigated in the guinea-pig isolated small intestine. Peristalsis was induced by raising the intraluminal pressure and the volume of fluid propelled was taken as a measure of the efficiency of peristaltic activity. 2. Low concentrations of pirenzepine (0.1-1 nM) and of hyoscine (0.01 nM) significantly enhanced peristalsis, whereas larger concentrations of both drugs caused inhibition. Pirenzepine was about 6 times less potent than hyoscine in increasing peristalsis, but was about 100 times less potent in inhibiting it. 3. We investigated the role of GABA on intestinal motility using as model the murine distal colon. Effects induced by GABA receptors recruitment were examined in whole colonic segments and isolated circular muscle preparations to analyze their influence on peristaltic reflex and on spontaneous and neurally-evoked contractions. Using a modified Trendelenburg set-up, rhythmic peristaltic contractions were evoked by gradual distension of the colonic segments. A single injection of azadirachtin into nymphal and adult locusts results in a decrease of food intake but not in reduced food utilization. Feeding deterrency is correlated with a gradual inhibition of midgut peristalsis. Gut movement is under the control of the stomatogastric nervous system, especially the ingluvial ganglion, as demonstrated in anin vitro system. It is shown that the peristaltic activity of the isolated mesenterial-lymph vessels of mammals (Cavia porcellus L.) is provoked by two different stimuli: 1. by extension (internal pressure of the vessel: 2–25 cm H2O); 2. by temperature (biokinetic Temperature-Space of 23°; between 22–45°C). Based on these experiments, the autochthonous automaticy of the lymph vessels has been proved.

AbstractThe role of both peripheral and central mechanism in the control of primary peristalsis was studied in pigeon cervical oesophagus.The results from the transection of oesophageal muscular wall and of extrinsic nerves suggest that :i) primary peristalsis is programmed centrally.ii) extrinsic motor input is carried in glossopharyngeal nerves and distributed separately at each oesophageal level through intramural neurons.iii) intramural neurons do not seem capable of propagating the peristaltic sequence irrespective of the central control.
Alteration of Esophageal Peristalsis by Pentagastrin in Patients with Diffuse Esophageal Spasm

Although it has been shown that gastrin and gastric alkalinization affect the lower esophageal sphincter, in vivo studies have not demonstrated a measurable effect of pentagastrin on esophageal peristalsis. In 9 patients with diffuse esophageal spam and in 10 control subjects esophageal peristalsis was recorded before and after pentagastrin infections. Subcutaneous pentagastrin increased peak amplitude significantly more in patients, 31.2 +/- 8.1 mm Hg (mean +/- S.E.M.), than in controls, 12.1 +/- 5.1 mm Hg (P less than 0.02).
