Pulmonary hypertension (PH) is a disease group that includes a wide variety of entities leading to an increased pulmonary arterial pressure. This chapter describes the basic mechanisms that lead to PH and the possibilities of MRI in diagnosing different aspects. A MR imaging protocol is provided making MRI a comprehensive modality for the classification of the underlying disease and assessment of hemodynamics.

Objectives There is a need for implementing the knowledge of cardiovascular (CV) risk in patients with rheumatoid arthritis (RA) into clinical practice. Our aim was to evaluate CV risk factor (CVRF) recording in a rheumatology outpatient clinic (ROC), where the standard was annual CVRF recording. A second aim was to compare CVRF recording between a regular ROC (RegROC) and an arthritis clinic (AC), which consisted of a structured, team-based model. “Small volume resuscitation” (SVR) is a promising concept for the treatment of shock and trauma patients. SVR utilizes the fast infusion of a small volume of hypertonic saline to mobilize intraendothelial and parenchymal water to expand and restitute intravascular volume. Therefore it seems warranted to also consider SVR for the treatment of disturbances of the cerebral circulation and of increased intracranial pressure (ICP). Pulmonary thrombendarterectomy was performed in 32 patients (14 men and 18 women; mean age 38 +/- 15 years) with thromboembolic pulmonary hypertension (New York Heart Association stage III: n = 22; stage IV: n = 10). The preoperative arterial pO2 averaged 59 +/- 11 mm Hg; pulmonary vascular resistance (PVR) and mean pressure (MPAP) were increased to 1,045 +/- 430 dyn.s.cm-5 and 53 +/- 12 mm Hg, respectively. The perioperative death rate was 22% (7 of 32). In the 25 survivors the pulmonary hypertension was reduced to a PVR of 194 +/- 75 dys.s.cm-5, MPAP of 28 +/- 6 mm Hg.
